Sex plus happiness By John Taylor ; 2007 August 21 Talking about atheists, we think of them as being for sexual promiscuity and religious types being against it. Not always. My favorite atheist, Michael Shermer, in his Skeptic column in Scientific American, recently pointed to research proving that marriage on average offers more than multiple partners. Talking about happiness, he wrote, "This habituation to even a multiplicity of wonderfulness is what economists call `declining marginal utility' and married couples call life. But if you think that an array of sexual partners adds to the spice of life, you are mistaken: according to an exhaustive study published in The Social Organization of Sexuality (University of Chicago Press, 1994), married people have more sex than singles -- and more orgasms." (Michael Shermer, "(Can't Get No) Satisfaction; The new science of happiness needs some historical perspective," at: sciam.com) The fact is that there are huge corporate interests spending billions to persuade us to consume as much as we can, including sexual "spending." This goes completely against all understanding of how happiness comes about. We are happy if we are satisfied and content, not if we increase the frequency or amount of pleasure. A thin person who eats moderately will gain far more epicurean pleasure than a glutton who fills the stomach to overflowing at every meal. The same thing is true for sex. Take Viagra, for example. In 1999 it was introduced along with a massive publicity campaign. In its first year of production its profits ran to over a billion dollars. That is not chicken feed, even by corporate standards. Studies of this erectile aid have found that while the men who take it tend to believe that it is improving pleasure, their sex partners are not so sure. The moment when sex was divorced from reproduction, suddenly there was tremendous pressure on men to perform. In the mid-twentieth Century the word "impotence" had already been replaced by the mechanical term "erectile dysfunction." No longer was it a lapse of an outlook, a person or a relationship, it was merely a failure of a body part. Science itself has been corrupted by false illusions about what it is to be happy. Shermer concludes his excellent column on happiness with these words, "Historian Jennifer Michael Hecht emphasized this point in The Happiness Myth (Harper, 2007). Her deep and thoughtful historical perspective demonstrates just how time- and culture-dependent is all this happiness research. As she writes, `The basic modern assumptions about how to be happy are nonsense.' Take sex. `A century ago, an average man who had not had sex in three years might have felt proud of his health and forbearance, and a woman might have praised herself for the health and happiness benefits of ten years of abstinence.' Most happiness research is based on self-reported data, and Hecht's point is that people a century ago would most likely have answered questions on a happiness survey very differently than they do today. To understand happiness, we need both history and science." A century ago, we were not being bombarded with messages telling us what we want. Or what will make me happy. Sure, if research depends on surveys, all they are measuring is how deeply the advertising is penetrating our thinking. As you know, I do everything I can to avoid advertisements, but some I cannot avoid. Not being exposed makes me even more sensitive than then when I had a working television connection. A television advertisement now hits me in the face like a sledgehammer. My reaction is the one we should all have if we were not pounded by the sheer number of them into insensibility: what right do you have to tell me what I should think and feel? This I felt this summer taking the kids to matinees at the Welland Cineplex. Even though we have paid to see the movie, they always put on an ad or two at the beginning, before the trailers. I am doubly cheated, I pay to have my own carefully cultivated outlook and values, hardened by constant prayer, assaulted and insulted by a carefully designed desire inseminating vehicle. Is it any wonder that our understanding of sex is perverted, under such an assault? We must have sex, and the more the better. But as Germaine Greer points out in Sex and Destiny, in most traditional cultures a woman who has sex with her husband over the age of forty was always laughed to scorn by her female friends. What is the point of having sex at such an advanced age? Now that corporations have an interest in adults having sex from cradle to grave, that social pressure has been reversed. Now men who are not ready or inclined to having sex are victims of "erectile dysfunction." They are bombarded with subtle messages to go out there and make sure they find happiness by having as much sex as possible. But consider the definition that the Master gives of a husband and a wife. Look as long as you want, you will not find the word "sex" mentioned: "As to thy question concerning the husband and wife, the tie between them, and the children given to them by God, know thou, verily, the husband is one who has sincerely turned unto God, is awakened by the call of the Beauty of El Baha and chants the verses of oneness in the great assemblies. The wife is a being who wishes to be overflowing with and seeks after the attributes of God and His names; and the tie between them is no other than the Word of God." (SW, Vol. 9, p. 85) Not sex, the Word of God. Here is the rest of this Tablet. Search all you want, you will not find sex mentioned here either. "Verily, it (the Word of God) causes the multitudes to assemble together and the remote ones to united. Thus the husband and wife are brought into affinity, are united and harmonized, even as though they were one person. Through their mutual union, companionship and love great results are produced in the world, both material and spiritual. The spiritual result is the appearance of divine bounties. The material result is the children who are born in the cradle of the love of God, who are nurtured by the breast of the knowledge of God, are brought up in the bosom of the gift of God and are fostered in the lap of the training of God. Such children are those of whom it was said by Christ: "Verily, they are the children of the Kingdom.” ( Id. )
Nancy Hamant on Medicare Advantage: "What a crock!"
Posted on June 20, 2008 in Generic prescription drug list
From Nancy Hamant, May 23, 2007 Subject: Fwd: Medicare privatization and where are the press when you need them? It appears that part of the monthly Medicare premium of $93.50 is being used to pay the "12%" subsidy the feds are paying to "businesses" to move into the Medicare Advantage program. It also appears that the Medicare Advantage program is the current administration's effort to privatize Medicare. Also, the Medicare Advantage programs will eventually cost more! What a crock! Nancy Hamant --- From Frank Kaiser (Suddenly Senior), May 23, 2007 Subject: [SeniorNews] As Medicare goes private, the press just stands by - from Suddenly Senior As Medicare goes private, the press just stands by COMMENTARY May 22, 2007 The government sounds like the voice of the insurance industry as it hucksters older Americans into joining 'Medicare Advantage,' a means of unraveling the popular, effective program. Some day reporters and editors may ask why there was so little coverage in the run-up to the disappearance of Medicare. By Gilbert Cranberg Des Moines Register and Tribune. gilcranberg@yahoo.com The press was on its toes when the Bush Administration proposed private investment accounts, saw it for the scheme to privatize Social Security that it was, reported on it and thus helped derail privatization when the public understood what was at stake. Not so with the administration's plan to privatize Medicare. Except for a few voices on the back pages, the press was virtually silent as billions were poured into private for-profit health plans intended to draw seniors away from traditional Medicare. Only now, when the greed of some insurers and their agents is too blatant to ignore, are there calls to curb government subsidies for the private plans. Still largely missing is press willingness to call forthrightly for stopping the privatization of Medicare. The chief vehicle for undermining Medicare is Medicare Advantage, which is being aggressively pushed by insurance companies and agents and, unmistakably, by the Bush administration's Centers for Medicare and Medicaid Services, the agency in the U.S. Department of Health and Human Services that oversees Medicare. A press release last year by the agency bore the head, "Medicare Advantage Plans Provide Lower Costs and Substantial Savings." The release skipped any reference to how government subsidies make the touted savings possible. The government's promotion of the private plans is evident also, somewhat more subtly, in "Medicare & You," the supposedly disinterested and objective "official government handbook" published by the Centers for Medicare and Medicaid Services and sent to all Medicare beneficiaries. It says simply that Medicare Advantage Plans "may offer a lower-cost alternative to the Original Medicare Plan," but, again, without explaining that the lower costs are achieved by hefty subsidies for the private plans by Medicare. Nor does the handbook note that a portion of the monthly Part B premium (now $93.50) seniors pay for physician services helps underwrite the subsidy. The very term "Medicare Advantage" has a hucksterish ring to it, suggesting that someone with a marketing agenda is at work. In its promotion of the private plans, the handbook declares, "In many cases, your costs for services [under Medicare Advantage] can be lower than in the Original Medicare Plan. Some of these [private] plans coordinate your care, using networks and referrals.... This can help manage your overall care and can also result in savings to you." The handbook generally downplays the cost of co-pays. Medicare is stunningly successful and popular. Why would anyone want to desert it? Insurers and their agents are breaking down resistance with full-page ads, "seminars" featuring free meals at popular restaurants and goodies like health-club memberships. Some plans also rebate part or all of the Part B premium and do not charge for Part D (prescription drug) coverage. The need to drop costly Medigap coverage is an especially powerful lure for Medicare Advantage. Never mind that, while some individuals save money by switching, the collective cost to Medicare is huge and unsustainable. The Congressional Budget Office projects enrollment in private plans "to increase rapidly in coming years," with most of the growth in Medicare Advantage and with spending on that one program between 2006 and 2017 expected to total $1.5 trillion. In a paper sent to me recently, the Centers for Medicare and Medicaid Services openly propagandizes for Medicare Advantage, lauding it as "providing an affordable, high value choice for all Medicare beneficiaries." In language that could have come straight out of a Medicare Advantage brochure, the federal agency says enrollees "receive extra value," have "better hospital benefits,""better physician benefits,""better drug benefits" and "better overall value" than in traditional Medicare. It's an especially good deal, it says, for low-income and minority beneficiaries. Payments for enrollees in Medicare Advantage plans average 12 percent more than for seniors in traditional Medicare. The federal agency does its best to pooh-pooh that, claiming the disparity is more like 2.8 percent. Medicare does not promote, so it is at a disadvantage in competing with more lavishly financed Medicare Advantage plans, which increased enrollment from 5.3 million in 2003 to 8.3 million last February. Call traditional Medicare Medicare Disadvantage. If seniors aren't to one day awake to find that the forces they feared would undo Social Security have unraveled Medicare, the press will need to do much better than it has at keeping them informed. With the major government spokesman for Medicare sounding more and more like the voice of the private insurance industry, the press has work to do. Honda, Hybrids and Diesels

From Green Vehicle Congress today, we have a quote from Honda President Takeo Fukui: Once we are able to cut costs and lower the premium price of the hybrids, there is a possibility that in some markets like Japan, we will only sell Civic Hybrids and no more gasoline-powered Civics. We plan to install diesels in bigger models and hybrids in smaller models. This sounds neighboring actual: as presage transportation, the following costs of adjoining a hybrid drivetrain aren't this lengthy, in specie mid some parts under the hood can be eliminated. Epidural (Cont-1)
Posted on June 10, 2008 in Ed pump
Question: What are the indications for epidural anaesthesia? Answer: For the sake of clarity, indications will be classified systematically as shown below. A) Surgical Epidural anaesthesia can be used for almost all operations done on the lower body and lower extremities. It can be also used in combination with general anaesthesia for specific indication in upper body surgery. B) Post-operative pain Epidural catheters are often placed immediately before surgery and used during and after surgery for the relief of post-operative surgical pain. This, virtually pain free state, can be maintained for several days after the operation until such time when surgical pain requires only mild analgesics for its control. Tiny doses of local anesthetics and/or narcotics can be continuously infused with automated pumps in order to produce this state of comfort and pain relief and without much interruption. Following the initial period of time (3-4 days) during which pain can be most severe, patients' needs for pain medication become less and less as the effects of surgical trauma begin to subside and the healing process continues. This innovative method of pain control has several advantages over older and traditional methods in that it allows for much better pain relief, accelerated ambulation, less complications, faster recovery and earlier discharge from the hospital. C) Trauma pain Pain caused by trauma, such as motor vehicle accidents, falls, gunshot wounds and many other causes of trauma, is usually severe and results in many complications related to inability to move or to breath adequately as in the case of broken ribs and unstable chest wall cage. If untreated, it can result in severe disabilities and complications. Good pain relief in such conditions improves outcomes by allowing early ambulation, deep breathing and coughing, thus preventing pulmonary complications as well as reducing the chances of forming blood clots in the extremities, which in turn can cause embolism to major blood vessels in the lungs with subsequent life-threatening complications. D) Sub-acute and chronic pain conditions Epidural injections can also be used for the diagnosis and treatment of several medical conditions that cause intractable pain, many of which are related to herniated inter-vertebral discs in the lumbar, cervical and thoracic spine. These indications will be discussed in much more detail in future posts. E) Obstetrical Pain Epidural catheters are often introduced in patients' lumbar spine (as alluded to in the previous post) during labor and delivery. Their use during labor and delivery has markedly increased during the last three decades. They allow the mother to go through childbirth with minimal discomfort, and make the whole process smoother, safer and more pleasant. Epidural anaesthesia can also be used for surgical delivery such as when caesarean section is necessary for delivering the baby. More will be written on this in future posts.

I swear to God if I see one more kid out of a seatbelt, I'm going to go vigilante on somebody! I see it every day when I pick up my son from school. Kids hanging out of windows, kids riding in the back of pickup trucks, (yeehaw!) and basically jumping around in the car. It just drives me absolutely, frigging batty!!!!!!! Once when I was pregnant with my youngest, I was sitting in my car waiting for my husband to come out of a store when this chick pulled up in a Trans Am with two kids sitting in the front passenger seat. One was about 4 years old and the other was under a year old. I said pleasantly of course, "where's the car seats?" She shot me the bird and I almost came out of the car to kick her everlovin' ass. Ooppss, I remembered I was pregnant. Damn! "Did you know... During 2003, 8,089 passenger vehicle occupants under 15 years old were involved in fatal crashes. For those children, where restraint use was known, 30 percent were unrestrained; among those who were fatally injured, 53 percent were unrestrained. (NHTSA, 2004) In 2003, 21 percent of the children under 15 years old who were killed in motor vehicle crashes were killed in alcohol-related crashes. (NHTSA, 2004) Of the children 0-14 years old who were killed in alcohol-related crashes during 2003, 47 percent (209) were passengers in vehicles with drivers who had been drinking. (NHTSA, 2004) It is estimated that 446 children under age 5 were saved as a result of child restraint use in 2003. An estimated 7,020 lives were saved by child restraints from 1975-2003. (NHTSA, 2004) Motor vehicle crashes are the leading cause of death for children from two to 14 years of age. (NHTSA, 2003) " Please buckle up your children if you love them! I don't want to read in the obituaries about the fatalities. Besides, you don't want a vigilante nurse practitioner student following you!

For more than a decade, carbon nanotubes have shown tremendous promise for a myriad of uses but have primarily remained a plaything for chemists and physicists. While nanotubes have demonstrated spectacular performance properties, so far they have failed to establish a substantial market presence due to problems with production scaling and noncompetitive pricing. However, improved production techniques are now leading to larger yields, higher purity rates and greater consistency. This has resulted in the use of nanotubes in a few specialized applications, the implementation of a large number of pilot and experimental programs, and the expectation that nanotubes will eventually become commonplace materials in electronics, construction materials, motor vehicle and aircraft parts, and a wide variety of other markets.Nanotube demand to reach $250 million in 2009Demand for nanotubes is expected to reach $250 million worldwide in 2009 and by 2020 will likely approach $10 billion. (This forecast assumes continued success in production technology improvements to bring pricing to a competitive level.) Early successes will be in electronics, where performanceenhancing attributes offered by nanotubes will allow manufacturers of displays and other electronic components to meet increasingly demanding goals. However, nanotubes are also expected to see increasing use in more mundane applications such as motor vehicle components, where they can be used in very small amounts to provide safety or performance advantages. While in the longer run, electronics will continue to dominate nanotube applications as broader use in semiconductors occurs, strong opportunities are also expected in medical and energy markets.Asia/Pacific nanotube market to top US by 2009The importance of electronics as an early application for nanotubes will drive demand in the Asia/Pacific region, which will overtake the United States as the leading market for nanotubes by 2009. Strong growth is expected in South Korea, Japan and China. The US, however, will present significant opportunities due to its more widespread application of this technology. Air and water

Last weekend I took some advice often given to bloggers and got out more. Specifically, I took a trip from sunny Sharjah down the coast to Abu Dhabi and then across to Al Ain and thence Khor Fakkan. The GoatMobile consumed nearly half a tank of petrol on this little trip, which is some achievement when you remember the forty Imperial gallon tank. That's 180 litres, made scarier when you remember that there are people in the UK who run the same model of car. Ouch, expense. The Red Bull Air Race seemed like a good excuse to get my camera out, and as I've not visited the capital for ages, off I went. Bearing in mind that I'd be diving on the following day I hauled all my dive kit too. Traffic on Abu Dhabi corniche was predictably chaotic. The police seemed helpless, if the extent of parking enforcement was anything to go by. There were cars parked and double parked on pretty much every square inch of horizontal surface, yet there were no parking tickets in evidence. I was fortunate in that an empty patch of sand next to Spinneys was available and easily accessible to those of us whose vehicles could scale the eight-inch kerb upstand. Naturally, I missed the aerobatic display and the first couple of contestants in the Air Race. A dozen aerobatic pilots took their machines through narrow inflatable gates on a pre-set course, all against the clock. Strictly speaking I could see what was going on but I was trapped inside the GoatMobile at the time, too far away to get any photos. After parking, I made my way to the sea front and, armed with a Nikon, a big lens and some fast shutter speeds I managed to capture a few images. Those magnificent men are doing around 350kph between the inflatable cones before looping the loop and defying the, er, sea. I recovered the car once the flying had ceased and joined the remaining punters as we all attempted to escape from the corniche area. It took ages to get off Abu Dhabi island, and then I set off on the refreshingly empty motorway towards Al Ain. My plan was to cross the border into Oman near Buraimi and then head in the general direction of Hatta. I've not been to Al Ain for ages either. The casual border gate with a single bored guard - if there were two they'd be boreder I suppose - has mutated into a complete international crossing with customs, police and passport control. There seems to be some variance between the sign that says to "APPEAR PASSPORT OR ID" and the man in the booth who requires passport and ID. Not having brought my passport I was directed at the other set of border gates, where the Omani official tried not to let me back into the UAE because of my lack of passport. "But that's why they won't let me leave. So I'm not entering the UAE because I never left." Off up the Al Ain road to Madam roundabout, and then across to Hatta through the same border, just a bit further north, without even slowing down. Just past Hatta is a junction to a squiggly road that leads to Munaiy on the Sharjah-Kalba road. Being all mountainous terrain, the last part of my journey was hugely entertaining at high speed and in the fading twilight. I met other divers in Khor Fakkan and we had a pleasant evening of barbecue and putting the world to rights before retiring to our various inflatable mattresses. Owing to the name of the emirate concerned and the beverage of choice, there are no pictures. The diving on Saturday was very refreshing. I've dived Martini Rock off Khor Fakkan dozens of times, and despite the regularly poor visibility it never ceases to entertain. But I've not dived Inchcape 10 before. Lying just off Fujairah, I hope to dive it a lot more. The wreck is teeming with life. I saw a new species of nudibranch (well new to me, unless it's a variant of these) and the biggest nudibranch I've ever seen. Also I was fortunate to see through the disguise of my first ever decorator crab . The moray , hiding in an old tyre, was crying out to be photographed. The water temperature is still a little chilly. It's in the low to mid twenties Celsius. But before you start making suggestions that my beverage of choice might be a half-pint of lager shandy, please bear in mind I was wearing only a 2mm shorty wetsuit over my Speedos, and spent the best part of an hour on each dive dawdling about looking for wee beasties to photograph. Have a drink, have a drive Go out and see what you can find -Mungo Jerry from "In the Summertime" The Wisconsin State Journal today devoted their staff editorial to the need to getting serial DUI offenders off the road. To recap... Shockingly, Thomas J. Dworak has been convicted a dozen times for drunken driving. And he was in court this week standing trial for another drinking while driving offense. William A. Skare has been convicted on fourteen counts of drinking and driving. Clearly, these two Wisconsin men should not be allowed behind the wheel. And yet law enforcement keeps finding them there without a license and drunk. This constitutes a deficiency in our laws since the legal punishment for their repeated convictions has not prevented these individuals from driving. The Wisconsin State Journal puts it more eloquently, "The only regret Wisconsin should have about throwing the book at Dworak is that it is not a bigger book." Dworak is facing a maximum prison sentence of six years plus a fine of $10,000, which could increase as a function of his blood alcohol content at the time of the infraction. Hot off setting myself up for being called a yankee muckraker, Pint and Fork proposes that the following ideas be considered to keep serial offenders off the road: 1. Pass a law that makes it illegal for repeat offenders to own or possess a car after a certain number of offenses. I mean if we can make laws that "infringe" upon a convicted felon's "second amendment right" to bear arms, we can make a law that restricts access to motor vehicles. Cars in the hands of the intoxicated are deadly weapons and killed nearly 17,000 people in 2005 alone (insert obligatory comment about the number of US soldiers killed in the Iraq War, or on September 11, 2001). If we can keep the worst offenders away from motor vehicles, maybe we can reduce the number of alcohol-related fatalities. 2. Impose criminal penalties for allowing a known repeat offender to operate a vehicle in your possession. In my mind, this is similar to laws that we have in place concerning the provision of alcohol to minors. Law enforcement doesn't just penalize the offending minors; they can also penalize those who provided the alcohol in the first place. And so it is with cars and repeat DUI offenders. 3. The Wisconsin State Journal cites South Dakota law that allows repeat offenders to drive if they submit to Breathalyzer tests twice daily with the sheriff. I'm not sure that the article makes it clear, but the South Dakota law is a so-called "24/7 sobriety" zero-tolerance law. That is, a person can get a special permit to operate a motor vehicle so long as they get tested, pay the expenses of the testing, and consume absolutely no ethanol. The detection of any amount of alcohol necessarily revokes that individual's right to operate a motor vehicle. However unlikely, it may be possible to fool a Breathalyzer. Google helped me find all sorts of tricks for beating a Breathalyzer including breathing heavily before analysis, eating shit, and sucking on activated charcoal. My scientific training and an episode of Mythbusters has me doubting the effectiveness of any way to cheat the test. To obviate any technique for fooling a Breathalyzer analysis, blood samples should be randomly collected from program participants. I'm not sure that measure three would stop Dworak from driving, seeing as how he wasn't deterred by having his license revoked by a preponderance of repeat offenses. So while this 24/7 sobriety program offers a legal road map to obtaining a driving permit, it is insufficient to keep the worst offenders from driving because they will do so with or without a permit. Still, I regard this as a significant step forward. cialis generic viagra online buy cilais viagra
