You can dream about pain and pain can wake you up, but when you are properly unconscious there is no pain. Unfortunately, chronic pain is often fairly mysterious. Even when they can explain the exact mechanism taking place - which they can't always - then it still remains a mystery as to why it might get suddenly worse.
