And on the subject of blindness, what about the widespread but too little known disease called AMD (Age-Related Macular Degeneration) -- the leading cause of blindness in people over 50. Consider the Baby Boomers, the largest population bulge in history. Closing in on 50, many already older, they're going to go blind in record numbers. The cure is out there, but pushed back for how many years because drug companies are chasing ED profits, not AMD antidotes? Ask them, ask yourself, which cure would you rather wait for? In theory, maybe a tough call. In practice, no contest. Here's an even more troubling question: if Erectile Dysfunction drugs came from stem cell research, would George W, Tom DeLay, Bill Frist and the rest of the MEN in that crowd be opposing it so vigorously? A letter in Psychiatric News discusses the problems with the mental status examination: The MSE, often characterized as psychiatry's equivalent of the physical exam, is in fact a mixture of historical information, observations, and conclusions. "Thought process" is known from examination, but what we describe as "thought content" is history—phobias, compulsive behavior, and suicide ideation, for example. Hallucinations occurring at the time of the examination are "current mental state"; yesterday's hallucinations are history. "Judgment" is an evaluation. The MSE is flawed because of a lack of agreement about the meaning of some of the terms. "Mood" and "affect" are often confounded. "Orientation to person" refers to the patient's awareness of his or her own identity, but some take it to mean recognition of the examiner, while "orientation to situation" is not a standard question. "Judgment" may reflect the patient's answers to test questions, or it may reflect recent conduct, such as fighting with a police officer or giving money to a con man. Some describe a patient as having impaired judgment if he or she has a drug habit or stops taking prescribed medication. Note to medical students: the mental status examination is an examination of the patient's complete mental state, including mood, organization of thoughts, abnormal perceptions (hallucinations), cognition, etc. This is different from the Folstein Mini-mental status exam, which is a 30 point scale that measures only cognition. Araujo's family members told the media after his death that immigration authorities ignored warnings that the 34-year-old had epilepsy and needed medication. Lizbeth Morales, a niece of Contreras-Dominguez, said her family was trying to cope with news of her death and waiting for results of the autopsy report. An attorney representing one of Contreras-Dominguez's cellmates called Morales on Wednesday, she said, and reported that her aunt had complained of leg pain before her death, raising questions about the quality of medical care. "I don't know what really happened in there," she said. "I've thought about filing a complaint," Morales said, but decided to wait for more information. "Nothing is going to bring my aunt back," she added. "It's just sad." The deaths come amid growing scrutiny of the health and medical care provided to immigration detainees. Class-action lawsuit In June, the American Civil Liberties Union filed a class-action lawsuit on behalf of immigrant detainees at the San Diego Correctional Facility, charging that inadequate medical and mental health care has led to unnecessary suffering and avoidable deaths. In one such case, according to the ACLU, a Ghanaian man suffering obvious chest pains was denied treatment and was ordered to submit a written sick call request shortly before his death. In Texas, the ACLU has sued over an ICE facility in Taylor for immigrant families, alleging inadequate health care and psychologically abusive guards. ICE says the facility, run by a private prison operator, is a humane alternative to separating parents and children as they fight deportation or seek asylum. An autopsy was scheduled for Contreras-Dominguez on Thursday, but the report was not yet ready to be released to the public, according to the El Paso Medical Examiner's Office. Leticia Zamarripa, an ICE spokeswoman, did not provide information on Thursday on the number of deaths in ICE custody in recent years. She said Contreras-Dominguez was given a full medical examination after she was taken into custody Aug. 1. The detainee was given "prenatal medication," Zamarripa said, but she did not know what specifically that was. History of blood clots At 8 p.m. Monday, Contreras-Dominguez went to the detention center's medical facility, where she was given a snack, according to ICE records. Zamarripa said Contreras-Dominguez then said for the first time that she had a history of blood clots during pregnancy and had pain behind her knee. Zamarripa said Contreras-Dominguez was taken Monday night to a hospital in El Paso and held overnight. On Tuesday, the detainee returned to the ICE center and was placed in medical housing unit for observation, Zamarripa said. At 8:22 p.m. Tuesday, officials at the immigration detention center called emergency services after Contreras-Dominguez lost consciousness. She was pronounced dead at 11:22 p.m. at a hospital in El Paso, Zamarripa said. 2005 drug conviction Contreras-Dominguez was stopped April 12, 2005, at the port of entry in El Paso, driving a 1999 Ford pickup. She had her five children in the car, according to court records. After a drug-sniffing dog found marijuana bundles in a spare tire, Contreras-Dominguez said she was told she would be paid $500 to smuggle the drugs across the border, records show. She pleaded guilty in November 2005 to illegally importing a controlled substance and was sentenced to 18 months in prison. After her release, she was picked up by an ICE fugitive team, and was facing deportation based on her drug conviction.

The Pakistan Peoples Party's charges against bodyguards of a provincial minister accused them of attempting to assassinate Dr. Azra Fazal, who escaped harm although her bullet-proof vehicle was damaged in the attack. Dr. Azra Fazal is the sister-in-law of Benazir Bhutto, former Prime Minister of Pakistan and chairwoman of the Pakistan Peoples Party. In other incidents, Parliament member Naheed Khan was fired upon, member Sherry Rahman was attacked with a blunt instrument, and councilor Dr. Shahida Rahmani was hospitalized after being manhandled by supporters of the Muhajir Qaumi Movement (MQM), coalition partner of the Pakistani government. Other Pakistan Peoples Party leaders were physically assaulted during local elections when government supporters grabbed ballot boxes to stuff them with votes. I've posted before. Additional Health Insurance in California The traditional health care delivery system is based on a fee-for-service type of arrangement. In a fee-for-service system, you pay or each itemized medical service you receive. In the days of the frontier, "Doc" often received a chicken as payment. Today, physicians are paid with money, lots and lots of it. Fee-for-service health insurance recognizes this practice and is designed to reduce or even eliminate your duty to pay directly for your medical care. Traditional health insurance comes in three parts: California has four basic options for choosing a health care plan: 1. Health through an employer or association 2. Health Insurance through Income eligibility such as Medicaid 3. Health care for high risk individuals such as those that have had cancer or a heart attack 4. Private Insurance Hospitalization Hospitalization covers defined expenses incurred while in the hospital. Generally, the insurance will pay for all of the covered services rendered by the hospital staff. However, if the insurance benefit is an indemnity payment, the payment will be for a fixed sum regardless of the actual expenses incurred. This fixed sum will usually be far below the daily charge actually made by the hospital. Medical/surgical This part of a traditional health plan covers the expensive costs of medical care other than the bill from the hospital. Services such as doctor visits, treatment charges, etc., are covered here. Medical/surgical usually has a deductible and requires co-payments by the insured (payments you make for charges not covered by the insurance), typically 20 percent of the doctor's fee. Catastrophic or major medical There are usually lifetime maximum payments that hospitalization and medical/surgical plans will pay, after which the well runs dry. Unfortunately, these maximums may not be sufficient to pay for all of the care required if a major illness or injury should strike, since such afflictions can eat up hundreds of thousands or even millions of dollars worth of health services. Thus, catastrophic coverage adds to your umbrella of protection in an amount sufficient to protect you from the horrendous expenses of such serious and prolonged illnesses. These policies also fill in some of the gaps not covered by hospitalization or medical/surgical. Health Maintenance Organizations or Private Insurance in California The health maintenance organization (HMO) is a relatively new player in the health insurance game, although it has been around in a limited fashion since the 1930s. The idea behind an HMO is to pay one premium and receive all of your health care at no or a nominal additional cost. The point is to save money compared to traditional health plans that cost more to purchase and require more out-of-pocket payments from the insured. What you, the insured, give in exchange for reduced cost is a substantial loss of your freedom to choose who will take care of your health needs. Preferred Provider Organizations Preferred provider organizations (PPOs) seek to give both the benefits of traditional health plans and the money savings of HMOs. They do this by paying higher benefits as a reward for your using the doctors or hospitals they preselect for that purpose. Disability Insurance Disability insurance does not pay for health care; rather it pays for lost wages caused by a disabling injury or illness. How Health Insurance Is Priced Ask anyone how health insurance is priced and you will get a simple answer: expensively! Beyond that, there are underwriting criteria used by health insurance providers, whether they are for-profit or, like Blue Shield/Blue Cross, nonprofit. Underwriting Criteria Age. The older you are, the more likely you are to get sick; therefore, the higher your health insurance premiums will be. Number of people covered. Many people buy family coverage rather than individual policies. This means that there will be adults as well as minor children protected by the same plan. Some companies will charge based on the size of the family. Others charge a basic family rate without regard to the number of members. Gender. Unlike life insurance, where women get the better end of the bargain than men, in health insurance women often pay higher premiums. This is based on health insurance industry statistics which indicate that the female of the species tends to need medical care more often than the male. Health history.
