Author: koolspaces.com Now, I am not a doctor and I don't play one on T.V. but after living with diabetes for seven years I've learned some things that I haven't seen while researching diabetes. Most of the things you learn are the basics, e.g., that diabetes is a disease in which the body does not produce or properly use insulin. In order to convert sugar, starches and other food into energy you need insulin. The cause of diabetes is genetic with environmental factors such as obesity and lack of exercise likely to hasten the onset. My hope is if you are having difficulty with some aspect of diabetes this article may provide some insight to you. As always, check with your doctor prior to incorporating any changes to your daily regimen as each person is unique and may or may not have success with my experiences. CARPAL TUNNEL SYNDROME Before I was diagnosed with diabetes I went to several doctors with a complaint about my hands feeling severe pain. I especially felt it during a swing of a golf club. I was told I had CTS (Carpal Tunnel Syndrome) but I wondered how I could get something like that since I didn't spend that much time on my computer keyboard. I was also skeptical of the diagnosis because the nurse practitioner treating me left the room before reaching a verdict then returns and tells me I have CTS. I guess she discussed my symptoms with the presiding doctor. Later that week, I read an article saying that if your doctor diagnoses you with CTS ask him to give you a simple blood test to rule out more serious diseases like cancer and diabetes. I showed the article to my doctor who agreed and lo and behold when the test came back it showed I had diabetes. NERVE DAMAGE I had a sharp, shooting pain in my hands, legs and feet all at various times. My doctor suggested exercise. I didn't like jogging because I seem to aggravate my right knee when I do run. I have easy access to a pool but don't really enjoy it enough but I liked the idea of riding a bicycle around my neighborhood. The fresh air, meeting neighbors and taking it light on my knee seemed appealing to me. The trick is to find some form of exercise you can tolerate and keep up with. If you don't like jogging chances are you won't maintain the exercise required. You need ninety minutes of exercise a week. I usually like to exercise during the late afternoon but because of a hot spell I decided to exercise during the morning to escape the hot weather. To my surprise my glucose readings were somewhat high before dinner. So I went back to exercising at 4 p.m. and my glucose reading fell back considerably. I reasoned that my biggest meal of the day was dinner and exercising a few hours before dinner helped regulate my glucose. Also, I noticed my readings weren't affected much when I exercised 15 minutes, or even twenty minutes. It seemed to me that I wasn't benefitting from that amount of time spent exercising. But when I extended it to 25 and especially 30 minutes, I noticed the difference. My appetite wasn't as ravenous and I felt better overall. But best of all, the nerve damage to my legs and hands were gone. No more numbness in those areas as well. The only complaint I had was the amount of time it took me to get there, 3 - 6 months. Aren't we all impatient? I was so encouraged by the results that exercising has become a way of life for me. Now I bicycle 3 to 4 miles per day, 5 days a week. In fact, on the days that I do miss my workout I feel guilty. INFECTIONS I recently had a continuous spell of high glucose readings of which I had no rational explanation as to why. My exercise regimen was usual and my eating habits and weight were the same. I began taking Glyburide once a day. Even that didn't work. Concerned, I made a doctor's appointment and thanks to my doctor asking questions (innocuous I thought at the time) it turned out that I had an infection on my arm. As soon as I took the antibiotics, my glucose levels returned to where they previously were. I cut the pills in half (from 5 milligrams to 2.5) because when I was first prescribed the Glyburide I was not told about the side effects of the medication. I remember walking in the park and feeling terrible. I thought I was going to die. I made it home and took a glucose reading and was alarmed that it was 35. (Normal is between 70 and 125.) I called my doctor and relayed what was going on and they advised me to get some sugar in my body. I did and felt better within minutes. Today, I keep tomato juice in my refrigerator in case I need it. Don't make the mistake I made and try to raise your blood sugar with food. Food will take 15 minutes to digest and may raise it over the level you want thereby negating your goal. After taking the Glyburide during my fight with that infection, I had an Hba1c test and I was surprised that my lipid profile improved considerably. Also, I am glaucoma-suspect and my eye pressure level was reduced 26%! Is there a correlation between Glyburide and my improvements in these areas -- I don't know but I anxiously await my next quarterly blood lipid test. There is a positive side to diabetes. Now I'm forced to exercise, watch what I eat and keep tabs on my weight. None of which I did before. The result from all this is a healthier lifestyle, less complications from diabetes and a longer lifespan. Diabetes can be manageable. Consider it a wakeup call to a healthier lifestyle. I can't stress enough that before you make any changes to your daily regimen to talk it over with your doctor. Did you see anyone die crashing into a big tv on sale at Wal-Mart? For me Thanksgiving weekend went smoothly. I went to my aunt and uncle's home near Green Bay last Thursday. We ate plenty of turkey, drank a little wine, and took an entire pumpkin pie home--that lasted only one day. After pouring through the ads I didn't find anything to get me out on Black Friday. Instead, I slept late and watched whatever college sporting event was on tv. Saturday was off to my aunt and uncle who live near New London for another turkey meal. The highlights there were meeting my cousin's girlfriend and tossing beanbags outside because the weather was so nice. Sunday was football even though the Packers weren't playing. The Patriots beating the Bears made up for my bad fantasy football showings. My Thanksgiving weekend turned out well. I kicked back with the family and ended up not eating too much.

Even though I was working during winter break, I still managed to enjoy my time off. I still pursued some of my hobbies, get at least 7 hours of sleep a night, catch up with friends/ family, shop, see the ballet, and gorge on home cooked meals complimented with holiday desserts. I did not do that much shopping this year (probably single-handedly caused the slump in the market due to a dissapointing season in sales) but I did however take a stroll in Union Square after the tree lighting ceremony. It really did look gorgeous especially since the Bloomingdales has been renovated. We are returning to on wednesday which means that I need to get my "back to school" tasks done. Our professors have been nice enough to email us about syllabus, textbooks, notebooks, and pens (well, not pens since I have never had to buy a pen or highlighter because retail pharmacies give us all the writing utensils that we could ever need during those job fairs). Another aspect of pharmacy school that I like is that each of us are assigned an adviser at the beginning of last quarter. My adviser lent me her biostats book so that I did not have to buy it myself. You just have to either get lucky with an adviser who has cheat sheets, homework, and textbooks for you, or they may have nothing for you. Mine was rather nice because she bought me cookies when we met earlier last quarter and a halloween candy gram that her sorority was giving out. Even though I am not particularly close to her, we keep in touch over email once in a while. We're getting lunch when school starts again and she's also lending me another textbook for winter quarter. I plan to do something nice for her for her advice and all. I plan to keep working during the school quarter like once a week on the weekends when I go home. I can make enough to pay off my rent every month (yeah!). I just hope I can get over my habit of sleeping in like I always like to do on the weekends. Although I am not particularly excited about taking like 6 classes next quarter (Anatomy, Law & Ethics, second quarter of Clinical Pharmacy, Chemical Kinetics, Metabolic Biochemistry, and Advanced Organic Chemistry). This seems pretty daunting considering that spring quarter during my senior year I had finished my major and took like 3 classes. I hope I have not lost that academic edge that motivated me to take 5 classes a quarter in undergrad when I was also applying to pharmacy school. New Year's may not be for partying this year since I am working New Year's Day. Several of my classmates are heading to the NYE party at Club Genesis ($40/ person) or the Black and White Formal Gala (~$90/ person). Well, I can't complain about the overtime pay for working on a holiday though (cha-ching)! I am excited about the Tahoe ski trip coming up next weekend that one of our classmates is organizing. We will be going snowshoeing, skiing/snowboarding, and shopping in Reno.

My eyes are closing on me but I just got to do some blogging now. Sleepiness can't stop me from doing anything. errr...unless I'm out of control and the only thing I will be doing is dozzzz off. Its a very extremely hot hot hot hot hot . Believe it or not, I bath like 5 times today. Confessions; I was a little sweaty. hehe... A little scrub under the arms and between those legs. There are lots of commotion going around. When I woke up this morning, theres a maid in my house. Doing her cleaning. I was quite shock actually as I'm still on my PJ's. How embarrassing. I'm kinda use to it anyway. Its not like I'm naked or anything. What the heck. My mom hired a maid just to clean up ONLY the fridge for a day. Like when do we care bout that. So as she was doing her thing, I went for a cold bath. Then monitored her as I was doing revision for my up coming exam. Its so round the corner but I'm so not prepared yet. sigh...sigh...sigh... I've covered Law subjects but have not touch business studies yet. But luckily I went for researches bout marketing and production. Interesting tho. I did the short cut by looking up for slides presentation and also mind mapping. That got me half way through. Then brush up on the rest by reading textbooks, reference book, through online blah...blah...blah. Hope that's enough. As I was saying, today is a special day. Both my parents called whole day. One after the other. I was so irritated. Showing my 'irritateness'(if theres such a word) to them , they called less. But still so annoyed. They were just checking out on the maid. Its kinda stupid tho. Coz my house is monitored by CCTV. 8 or 9 around my house. In and out. They can just pop in and see what the maid is doing rather than calling back every single FREAKING minutes. DAMN!!! Glad thats over. Later at night, my family and I went for our Saturday Family Night Dinner. Thats the only time we see each other. They are so busy with their works. Early in the morning, they went to work. I didn't even have a chance to say hi or good morning. Then late at night, they come home. I was either out or in my room. Barely got time to see or hear them. Maybe twice a week, if I'm lucky, I'm able to meet them. This had been going on since I'm young. But our relations are still good. Our communication is through the phone. Weird huh. Ok then. We went for Indonesia's cuisine at Crown Square for a change. I didn't catch the restoran's name. I went there for the food. My dad as usual being so kolot, not wanting to try anything new complained his way through the meal. Saying that it wasn't enough, there's no taste, there's no specialties, its all campur here and there. The old man just couldn't shut up. Being sarcastic I stated that its satisfying. I like the food so much. Its the truth. It was a little tasteless and I don't think thats a problem. Eating less salt will save your life. He is so 'Chinese'. Its a fact that most Chinese like adding too much salt in their food. The saltier it is, the better it taste. Kinda true. I like food which have lots of taste in it, but I prefer to have a 'light' meal. Not too greasy, salty, sweet, spicy...etc. But herbs I liky. Its taste so naturally sweet, salty, 'herby'. So we end our dinner and we went seperate ways. He went for his night life and I went to mine too. I joined the 2 Sharon. When I called out 'Sharon', both heads turn. I should NUMBER them.

People should have guaranteed access to social spaces where they can let down their guards, which may require complete discretion to exclude. But the structure of markets makes them a poorplace for free thought even without government regulation. Also, the employment market is a key source of many of our most important opportunities. Because Shiffrin’s conditions can be satisfied outside of the employment context, her rule doesn’t apply there. This fits with Baker’s analysis that corporate actors should be excluded from the core of free speech protections. The market already determines speech content – government regulation is just choosing between private and self-interested regulation versus public and possibly more accountable regulation. Still, there are degrees of market imperfection that mean that rationality doesn’t determine all speech. Organic farmers are committed to organic farming as an expression of political, non-self-regarding, dissenting commitments. Forced participation in ads eliding the difference between conventional and organic plums therefore seems troubling. Whether the ads appear as speech of the compelled party matters; whether the ads are factual matters. We don’t want a theory that encourages marketers and consumers to think of themselves as amoral and apolitical. We should recognize attempts to moralize the market from within. Some on the left are trying to do this, as are various religious groups. Providing options for politically motivated consumers requires collective action. Organic farmers are not best understood as amoral profit maximizers. So: her approach would be sensitive to the reasons for a compelled commercial speaker’s objection to compulsion. Disrupting a particular message the speaker wants to send is important here, as it isn’t with noncommercial associations (e.g., Hurley ). C. Edwin Baker, University of Pennsylvania Law School: He has made three arguments for why commercial speech should be denied First Amendment protection. For him, free speech is libertarian. Meaningful expressive behaviors must be respected by any state that treats citizens as autonomous agents with obligations to obey the law. (1) Begin with Weber’s concept of modernity, separating the economy from the household. The market dictates to all that they must act efficiently or fail. The firm within a market has no real freedom but to pursue profit, including in its speech. Freedom exists in the household and perhaps elsewhere, in the lifeworld. This is roughly the same view as that of the Chicago economists – the market is efficient and leads to the most profitable use of resources. It is also the same view as Marx had. Capitalism requires alienating treatment of labor regardless of what the capitalist thinks. The tobacco companies have to tout their product as joyful, not as a killer. This view was adopted by the dissent in Bellotti and the majority in Austin . Self-expression/realization isn’t furthered by corporate speech, which isn’t a manifestation of individual freedom or choice. (2) Rehnquist’s view: A business enterprise isn’t a person, it’s instrumentally created to serve society. Society should be able to limit it to serve social interests. Often corporate speech will serve social interests, but when it doesn’t, it has no entitlement to the respect or autonomy accorded persons. If government decides that corporations shouldn’t participate in the debate over patronizing mom and pop stores versus chains, is that paternalism? Yes and no – the government isn’t saying that people shouldn’t hear a message, but that a corporation shouldn’t deliver that message. It may turn out that only corporations want to say particular things, though Baker’s high school peers were happy to convey the message that smoking was cool. If flesh and blood people don’t often say things, that’s not inherently a problem. Not many people want to deny the Holocaust either. Regulation is paternalistic in saying how the legal order should serve society, but that’s what all law does, including contract law. (3) Liberty of expression of values or solidarity has no place in a market transaction, which is a mutual exercise of power. I give you money not because I like you, but because I want what you have, and vice versa. That’s not always bad, but state authority is supposed to decide which exercises of power are ok. Lochner was wrongly decided. Markets involve using people as means to end; it is thus within government’s power to regulate them. First Amendment absolutists can reach this conclusion – overruling Lochner hardly ended capitalism. Charles Fried, Harvard Law School: He couldn’t disagree with Baker more. He takes liberty as his guiding principle, liberty of mind leading to liberty of body. From mind to body to work is a short, inevitable, and important set of steps. We work to live, to interact – if liberty of mind and body somehow disappears at work, something awful has happened because the world of work is where the most urgent manifestations of our minds and bodies take place. (In my experience, we usually call that the boss, not the government.) Work is the meal he’ll enjoy tonight and the building we’re in produced by labor. Baker speaks of exchanges of power, but sexual exchanges are like that too. Are we all dominated by power in our professional lives? Compelled to make the most money? Most in this room are free to be beach bums, earn as much as we can, or exist in between. (Yes, we’re quite the representative bunch.) Thus, Fried doesn’t see the market as a radical discontinuity from life. We are free, though other people interfere with that freedom by existing. Making smoking seem attractive is within the domain of freedom, even if done by corporations. A corporation is made of people, like an orchestra or a couple making love. He would not reify it as anything else. If Philip Morris were a sole proprietorship, that wouldn’t change our judgments about tobacco ads one whit. (And, as they say, if my grandmother had wheels she’d be a wagon. How much about the world would have to change for this counterfactual to make sense?)

Miami Grille- Poway, California

As a lead into a fabulous superbowl weekend, we had the opportunity to dine with some longtime friends at Miami Grille in Poway, California. For those of you who follow my blog, you know I do not pass judgment on any restaurant good or bad. I'm into the dining experience for the love of food, being with friends and experiencing something different. This place was pretty crowded with a long wait for an indoor table. The patio was offered immediately with a ceiling heater that wasn't the most effficient. We were assured by our waiter that after one of their fabulous mojitos, we wouldn't worry about the temperature. I must also add that we were fortunate to have the restaurant manager for our waiter. Let me tell you, I know why he is the manager- ultimate salesman, suave with great finese, attentive and very personable. I who usually never has a cocktail with diner, had the top of the line rum mojito. There were three choices of rum. Regular, good and ultimate. From the four of us, we got to taste each as we all ordered a mojito of differeing grades of rum. G was not impressed as sweet drinks arn't his thing. We began with appetizers- "Miami Grille Especiales" for $14.95. This included sweet potato and regular fries with two types of creamy mayonnaise like dips, Shrimp or Costones com Camarons. (Costones were described a unripen plaintains, flattened and fried. Starchy with flavor uplifted by the sauce). Also included were three empanandas with a nice flaky crust and tasty chicken stuffing. Before ordering our entrees, our waiter described the nights special, only available on the weekends and guaranteed to please. Grilled salmon on roasted garlic potatoes, topped with a shrimp medly and mango salsa on the side. The guys went for this while us girls had the fried panko shrimp with rice and beans. Presentation was superb for both entrees. The shrimp were large and meaty, fried to perfection. Damn, fat is good). The guys raved about the salmon. Thick, tasty and with the mango salsa, they were in heaven. Great choice. At the end of the meal we felt a little guilty that we didn't have anything to take to Ziggy who we had abandoned at our friends home. Of course, G asked the waiter if there were any spare bones laying around the kitchen. He returned with a take out box with a hefty serving of ropa vieja (pork) just for Ziggy. That Ziggy is so lucky.

Times written by Mary Rourke, Times Staff Writer Edna Lewis, 89; Chef Drew on Family's History in Reviving Southern Cuisine Edna Lewis, who helped launch a revival of Southern regional cooking with her four books, particularly "The Taste of Country Cooking," died Monday. She was 89. Lewis died of natural causes in her sleep at her home in Decatur, Ga., Scott Peacock, a longtime friend and Lewis' housemate in recent years, told The Times. She had been in failing health for several years and suffered from dementia. The granddaughter of freed slaves in Freetown, a Virginia farming community, Lewis had an eclectic career working as a restaurant chef, a pheasant farmer and a cooking teacher, among other things. But her cookbooks brought her national recognition. Along with "The Taste of Country Cooking" in 1976, she wrote "The Edna Lewis Cookbook" in 1972 and "In Pursuit of Flavor" in 1988. She and Peacock wrote "The Gift of Southern Cooking" in 2003. "Edna was a very important voice for her knowledge of Virginia-style Southern food and cooking," Judith Jones, Lewis' editor at Alfred A. Knopf publishers, told The Times in 2003. "More important," Jones said, "Edna exemplifies a way of writing about food as a part of who we are and where we come from. It is food writing as memoir." Some food experts referred to Lewis as the leading African American female chef. Others placed her as the dean of all Southern cooking. Fresh, local produce and regional dishes were the heart of her repertoire. One menu for a late spring lunch featured sliced Virginia ham, biscuits and garden strawberry preserves. "Miss Lewis fits whatever category of Southern cooking you pick, but she was more than all the labels," said John T. Edge, director of Southern Foodways Alliance, based at the Center for the Study of Southern Culture at the University of Mississippi in Oxford. In several of her books, she wrote about her early years in Freetown. Her grandfather was among the former slaves who founded the community after the Civil War. Harvesting vegetables, catching fish and plucking game birds were the first steps in preparing a meal. "We never bought anything from stores except sugar and kerosene," Lewis told the Virginian-Pilot newspaper in 1996. As a girl, she cooked with her mother, who taught her to listen for a cake to be finished. "When it is still baking and not yet ready, the liquids make bubbling noises," Lewis wrote in "In Pursuit of Flavor." Lewis' father died when she was 9. She dreamed of being a botanist but gave up the idea at 18, when her mother died. She moved to New York City looking for work in the early 1940s. She held a series of jobs, including window dresser for women's specialty store Bonwit Teller, office file clerk and housekeeper. She often cooked for her friends. One of them, John Nicholson, owned an antique shop. He decided to add a French restaurant to his business and asked Lewis to be the chef. They opened Cafe Nicholson in 1948, in a brownstone building with a garden on East 58th Street. Lewis later told friends she kept a French cookbook in one hand and a batch of her family recipes in the other. "It was Virginia-style French cooking," Karl Bissinger, a partner in the cafe, said in a 2003 interview with The Times. "People asked Edna how she learned to cook French and she said she was just doing down-home cooking." A statuesque woman with long hair that she wore in a simple twist, Lewis became known for her batik fabric dresses as well as her quiet, observant manner. She rarely spoke of her personal life. She was proud of her heritage but showed it in subtle ways, Jones said. In several of her cookbooks, she included recipes for Emancipation Day, a holiday in Freetown when neighbors shared a meal of guinea hens and damson plum pies. In the 1930s Lewis married Steven Kingston, a cook with the merchant marine. They were political activists who joined the Communist Party. "I was a radical," Lewis told Bon Appetit magazine in November 2001. She worked in the office of the Daily Worker, the Communist newspaper. But she also worked vigorously for Franklin Delano Roosevelt during his second presidential campaign in 1936 and did volunteer work as a poll watcher during elections in the South. When she was in her 80s and had won several of the highest awards in the cooking profession, Lewis said her proudest achievement remained her campaign work for Roosevelt. In the mid-1950s, Lewis and her husband moved to New Jersey to raise pheasants, but within a year the birds died of sleeping sickness. Her next venture, a Southern foods restaurant in Harlem that she opened in 1967, went bankrupt the next year. "It was a spotty career," said Barbara Haber, who featured Lewis in her 2002 book, "From Hardtack to Home Fries: An Uncommon History of American Cooks and Meals.""If an opportunity came, Edna went with it," Haber said. But if you think that an array of sexual partners adds to the spice of life, you are mistaken: according to an exhaustive study published in The Social Organization of Sexuality (University of Chicago Press, 1994), married people have more sex than singles -- and more orgasms." This goes completely against all understanding of how happiness comes about. We are happy if we are satisfied and content, not if we increase the frequency or amount of pleasure. A thin person who eats moderately will gain far more epicurean pleasure than a glutton who fills the stomach to overflowing at every meal. The same thing is true for sex. Take Viagra, for example. In 1999 it was introduced along with a massive publicity campaign. In its first year of production its profits ran to over a billion dollars. That is not chicken feed, even by corporate standards. Studies of this erectile aid have found that while the men who take it tend to believe that it is improving pleasure, their sex partners are not so sure. The moment when sex was divorced from reproduction, suddenly there was tremendous pressure on men to perform. Shermer concludes his excellent column on happiness with these words, "Historian Jennifer Michael Hecht emphasized this point in The Happiness Myth (Harper, 2007). Her deep and thoughtful historical perspective demonstrates just how time- and culture-dependent is all this happiness research. As she writes, `The basic modern assumptions about how to be happy are nonsense.' Take sex. `A century ago, an average man who had not had sex in three years might have felt proud of his health and forbearance, and a woman might have praised herself for the health and happiness benefits of ten years of abstinence.' Most happiness research is based on self-reported data, and Hecht's point is that people a century ago would most likely have answered questions on a happiness survey very differently than they do today. To understand happiness, we need both history and science." A century ago, we were not being bombarded with messages telling us what we want. Or what will make me happy. Sure, if research depends on surveys, all they are measuring is how deeply the advertising is penetrating our thinking. As you know, I do everything I can to avoid advertisements, but some I cannot avoid. Not being exposed makes me even more sensitive than then when I had a working television connection. A television advertisement now hits me in the face like a sledgehammer. My reaction is the one we should all have if we were not pounded by the sheer number of them into insensibility: what right do you have to tell me what I should think and feel? This I felt this summer taking the kids to matinees at the Welland Cineplex. Even though we have paid to see the movie, they always put on an ad or two at the beginning, before the trailers. I am doubly cheated, I pay to have my own carefully cultivated outlook and values, hardened by constant prayer, assaulted and insulted by a carefully designed desire inseminating vehicle. Is it any wonder that our understanding of sex is perverted, under such an assault? We must have sex, and the more the better. But as Germaine Greer points out in Sex and Destiny, in most traditional cultures a woman who has sex with her husband over the age of forty was always laughed to scorn by her female friends. What is the point of having sex at such an advanced age? Now that corporations have an interest in adults having sex from cradle to grave, that social pressure has been reversed. Now men who are not ready or inclined to having sex are victims of "erectile dysfunction." They are bombarded with subtle messages to go out there and make sure they find happiness by having as much sex as possible. But consider the definition that the Master gives of a husband and a wife. By Gilbert Cranberg Des Moines Register and Tribune. gilcranberg@yahoo.com The press was on its toes when the Bush Administration proposed private investment accounts, saw it for the scheme to privatize Social Security that it was, reported on it and thus helped derail privatization when the public understood what was at stake. Not so with the administration's plan to privatize Medicare. Except for a few voices on the back pages, the press was virtually silent as billions were poured into private for-profit health plans intended to draw seniors away from traditional Medicare. Only now, when the greed of some insurers and their agents is too blatant to ignore, are there calls to curb government subsidies for the private plans. Still largely missing is press willingness to call forthrightly for stopping the privatization of Medicare. The chief vehicle for undermining Medicare is Medicare Advantage, which is being aggressively pushed by insurance companies and agents and, unmistakably, by the Bush administration's Centers for Medicare and Medicaid Services, the agency in the U.S. Department of Health and Human Services that oversees Medicare. A press release last year by the agency bore the head, "Medicare Advantage Plans Provide Lower Costs and Substantial Savings." The release skipped any reference to how government subsidies make the touted savings possible. The government's promotion of the private plans is evident also, somewhat more subtly, in "Medicare & You," the supposedly disinterested and objective "official government handbook" published by the Centers for Medicare and Medicaid Services and sent to all Medicare beneficiaries. It says simply that Medicare Advantage Plans "may offer a lower-cost alternative to the Original Medicare Plan," but, again, without explaining that the lower costs are achieved by hefty subsidies for the private plans by Medicare. Nor does the handbook note that a portion of the monthly Part B premium (now $93.50) seniors pay for physician services helps underwrite the subsidy. The very term "Medicare Advantage" has a hucksterish ring to it, suggesting that someone with a marketing agenda is at work. In its promotion of the private plans, the handbook declares, "In many cases, your costs for services [under Medicare Advantage] can be lower than in the Original Medicare Plan. Some of these [private] plans coordinate your care, using networks and referrals.... This can help manage your overall care and can also result in savings to you." The handbook generally downplays the cost of co-pays. Medicare is stunningly successful and popular. Why would anyone want to desert it? Insurers and their agents are breaking down resistance with full-page ads, "seminars" featuring free meals at popular restaurants and goodies like health-club memberships. Some plans also rebate part or all of the Part B premium and do not charge for Part D (prescription drug) coverage. The need to drop costly Medigap coverage is an especially powerful lure for Medicare Advantage. Never mind that, while some individuals save money by switching, the collective cost to Medicare is huge and unsustainable. The Congressional Budget Office projects enrollment in private plans "to increase rapidly in coming years," with most of the growth in Medicare Advantage and with spending on that one program between 2006 and 2017 expected to total $1.5 trillion. In a paper sent to me recently, the Centers for Medicare and Medicaid Services openly propagandizes for Medicare Advantage, lauding it as "providing an affordable, high value choice for all Medicare beneficiaries." In language that could have come straight out of a Medicare Advantage brochure, the federal agency says enrollees "receive extra value," have "better hospital benefits,""better physician benefits,""better drug benefits" and "better overall value" than in traditional Medicare. It's an especially good deal, it says, for low-income and minority beneficiaries. Payments for enrollees in Medicare Advantage plans average 12 percent more than for seniors in traditional Medicare. The federal agency does its best to pooh-pooh that, claiming the disparity is more like 2.8 percent. Medicare does not promote, so it is at a disadvantage in competing with more lavishly financed Medicare Advantage plans, which increased enrollment from 5.3 million in 2003 to 8.3 million last February. Call traditional Medicare Medicare Disadvantage. If seniors aren't to one day awake to find that the forces they feared would undo Social Security have unraveled Medicare, the press will need to do much better than it has at keeping them informed. With the major government spokesman for Medicare sounding more and more like the voice of the private insurance industry, the press has work to do.
