News - Feds Warn Against Risky Silicone 'Pump Parties'

Posted on October 06, 2008 in Ed pump

Found this on AOL. We have known that silicone gel injections were a dangerous proposition for years. Why is it that people still seek them? I remember as a resident a patient who had had silicone gel injected into her breasts. This left her with breasts filled with nodules that felt like rocks. These hurt her a great deal. We ended up reconstructing her breasts utilizing her tummy tissue (Free TRAM) following mastectomies. Her results were numb breasts, but the pain was gone. A high price to pay for being stylish. Please research that which seems "new and different" before having it. You might find that it is neither new nor different. It might also not be desirable. Definitions of 'euthanasia'

Posted on September 29, 2008 in Generic medical release

With so much talk about 'euthanasia' these days after the Terri Schiavo affair (March 2005), and the affirming decision of the US Supreme Court (January 2006) in the Oregon physician-assisted suicide case, it seems time -- in the interests of mutual understanding -- to define the principal words being used. Thus............. ASSISTED SUICIDE: Helping a person to end his or her life by request in order to end suffering.. (Rarely prosecuted and only lawful in Switzerland where the reasons must be altruistic.) PHYSICIAN-ASSISTED SUICIDE: Medical doctor helping patient to die by prescribing a lethal overdose. Patient can chose whether to drink it. (Lawful only in Oregon, Switzerland, Netherlands and Belgium.) EUTHANASIA: A broad, generic term meaning 'help with a good death.' VOLUNTARY EUTHANASIA: Death by lethal injection by doctor when requested by patient. (Only lawful in Belgium and the Netherlands for the terminally or hopelessly ill.) NON-VOLUNTARY EUTHANASIA: Using powerful drugs, doctor ends life of suffering, dying patient who is comatose. Illegal, but happens all the time, discreetly, in the interest of compassion. TERMINAL SEDATION: Upon patient request, doctor puts patient into deep sleep with medications, during which time the patient dies either of the underlying illness or starvation/dehydration. Widely practiced and generally accepted as ethical and lawful. MERCY KILLING: Taking the life of another person in the belief that this is a compassionate act because the ill person is unable to do so. Unlawful. Study shows monkeys become increasingly motivated to obtain nicotine

Posted on September 24, 2008 in Diabetes erectile dysfunction

Nicotine use is highly addictive in primates, say researchers who conducted an unusual study of squirrel monkeys. The study by researchers from the Centre for Addiction and Mental Health in Toronto and the U.S. National Institutes of Health examined the reinforcing effects of nicotine. It found that squirrel monkeys who could give themselves nicotine by pressing a lever initially used it very little - but over time developed a "high motivation" for using it. "The number of the lever presses that the monkey had to perform to get a single injection of nicotine progressively increased," said Dr. Bernard Le Foll, a CAMH scientist and associate professor at the University of Toronto. "We were able to measure the motivation to take nicotine ... This revealed a high motivation to take nicotine, with monkeys pressing up to 600 times to get a single injection of nicotine." A catheter was implanted into a vein of the animals. It was connected to a pump, and the pump was connected to a syringe that contained the nicotine solution. Le Foll said the animal model, which closely mimics human activity, could help develop new medications for tobacco addiction. "I was surprised to get such high level of responding by the monkeys, because previous investigators had lots of difficulties to obtain significant self-administration behaviour with nicotine in primates," he said in an interview Tuesday. "That is an indication that nicotine is a critical component of tobacco smoke and that it is the desire to obtain nicotine that is an important drive of smoking behaviour." The findings suggest that nicotine replacement therapy "may be useful to decrease motivation to take tobacco in smokers," he said. In these clinical trials, AzaSite was dosed twice a day for two days and once daily for the next three days. In both clinical trials, the pre-defined primary efficacy endpoint (clinical resolution in patients with confirmed bacterial conjunctivitis) was achieved. Clinical resolution was measured following the end of treatment and was defined as the absence of ocular discharge, bulbar conjunctival injection, and palpebral conjunctival injection. Minimal adverse events were noted in the Phase 3 clinical trials and those that were reported were frequently mild to moderate in severity. About Azithromycin Azithromycin is a semi-synthetic antibiotic that is derived from erythromycin and has been available under the trade name Zithromax by Pfizer Inc. since 1992. Streptomycin

Posted on July 04, 2008 in Antibiotic

http://en.wikipedia.org/wiki/Streptomycin antiobiotic drug--> comes from a gram-positive bacterium (Actinobacterium). Streptomycin is an antibiotic drug, the first of a class of drugs called aminoglycosides to be discovered, and was the first antibiotic remedy for tuberculosis. It is derived from the actinobacterium Streptomyces griseus. Streptomycin stops bacterial growth by damaging cell membranes and inhibiting protein synthesis. Specifically, it binds to the 23S rRNA molecule of the bacterial ribosome, which prevents the release of the growing protein (polypeptide chain). Humans have structurally different ribosomes than bacteria, thereby allowing the selectivity of this antibiotic for bacteria. Streptomycin cannot be given orally, but must be administered by regular intramuscular injection. [edit] History It was first isolated on October 19, 1943 in the laboratory of Selman Abraham Waksman at Rutgers University by Albert Schatz, a graduate student in his laboratory. Waksman and his laboratory discovered several antibiotics, including actinomycin, clavacin, streptothricin, streptomycin, grisein, neomycin, fradicin, candicidin, candidin, and others. Two of these, streptomycin and neomycin, found extensive application in the treatment of numerous infectious diseases. Streptomycin was the first antibiotic that could be used to cure the disease tuberculosis. Waksman is credited with having coined the term antibiotics. The details and credit for the discovery of streptomycin were strongly contested by Albert Schatz and resulted in litigation. The contention arose because Schatz was the graduate student in charge of performing the lab work on streptomycin; however, it was argued that he was using techniques, equipment and lab space of Waksman's while under Waksman's direction. There is contention as to whether or not Schatz should have been included in the Nobel Prize awarded in 1952. However, the committee stated that the Nobel Prize was awarded not only for the discovery of streptomycin but also for the development of the methods and techniques that led up to its discovery and the discovery of many other antibiotics. The litigation ended with a settlement for Schatz and the official decision that Waksman and Schatz would be considered co-discoverers of streptomycin. Schatz was awarded the Rutgers medal in 1994, at the age of 74. The controversy ultimately had a negative impact on the careers of both Waksman and Schatz and the controversy continues today.

Epidural (Cont-1)

Posted on June 10, 2008 in Ed pump

Question: What are the indications for epidural anaesthesia? Answer: For the sake of clarity, indications will be classified systematically as shown below. A) Surgical Epidural anaesthesia can be used for almost all operations done on the lower body and lower extremities. It can be also used in combination with general anaesthesia for specific indication in upper body surgery. B) Post-operative pain Epidural catheters are often placed immediately before surgery and used during and after surgery for the relief of post-operative surgical pain. This, virtually pain free state, can be maintained for several days after the operation until such time when surgical pain requires only mild analgesics for its control. Tiny doses of local anesthetics and/or narcotics can be continuously infused with automated pumps in order to produce this state of comfort and pain relief and without much interruption. Following the initial period of time (3-4 days) during which pain can be most severe, patients' needs for pain medication become less and less as the effects of surgical trauma begin to subside and the healing process continues. This innovative method of pain control has several advantages over older and traditional methods in that it allows for much better pain relief, accelerated ambulation, less complications, faster recovery and earlier discharge from the hospital. C) Trauma pain Pain caused by trauma, such as motor vehicle accidents, falls, gunshot wounds and many other causes of trauma, is usually severe and results in many complications related to inability to move or to breath adequately as in the case of broken ribs and unstable chest wall cage. If untreated, it can result in severe disabilities and complications. Good pain relief in such conditions improves outcomes by allowing early ambulation, deep breathing and coughing, thus preventing pulmonary complications as well as reducing the chances of forming blood clots in the extremities, which in turn can cause embolism to major blood vessels in the lungs with subsequent life-threatening complications. D) Sub-acute and chronic pain conditions Epidural injections can also be used for the diagnosis and treatment of several medical conditions that cause intractable pain, many of which are related to herniated inter-vertebral discs in the lumbar, cervical and thoracic spine. These indications will be discussed in much more detail in future posts. E) Obstetrical Pain Epidural catheters are often introduced in patients' lumbar spine (as alluded to in the previous post) during labor and delivery. Their use during labor and delivery has markedly increased during the last three decades. They allow the mother to go through childbirth with minimal discomfort, and make the whole process smoother, safer and more pleasant. Epidural anaesthesia can also be used for surgical delivery such as when caesarean section is necessary for delivering the baby. More will be written on this in future posts.

How enforcing prohibition creates street crime

Posted on May 25, 2008 in Canadian drugs

Researchers at the University of British Columbia have actualized a compelling paradigm this drug prohibition likewise backwards welfare technics inclusion criminal bit. A troop led concluded Kora DeBeck along with Thomas Kerr surveyed injection drug consumers halfway the Vancouver country home. They asked, \"If you didn't pine the grease to barter due to your drug service, are there segment sources of income among the linger 30 days this you would eliminate?\" Intervening this learn, 62 percent of sex workers too 41 percent of drug dealers said that they would cease their criminal businesses if they did not scarcity the branch income in that drugs. It may seem obvious that streetwalkers don't double their jobs. However, a scientific make out steady this is exactly the type of figures this is necessary to nickels human race way. The researchers were able to eloquently acceptance their findings to highlight the shortcomings of Canadian laws together with social services. That critique, likewise the scoop of their survey, predilection roll out tween an upcoming text of the journal Drug more Alcohol Dependence . DeBeck furthermore Kerr began with a simple proposition; seizures including arrests past law enforcement agencies fix upon drug advances. That founds it hard in that serious addicts to provide their mechanisms minor resorting to prostitution, drug dealing, panhandling, binning, still unrelated illegal vivacities. By disrupting drug markets and increasing risks involved in producing and distributing illegal substances, prohibition-based drug enforcement policies play a role in inflating drug prices, which in turn induces active IDU (injection drug users) with high intensity addictions to engage in prohibited income generating behavior to finance their drug use. While the ultimate objective of inducing high drug prices is to deter drug use, this analysis and a growing body of research indicates that the unintended consequences of these enforcement-based policies produce significant harm for drug using individuals and broader society. Here is the biggest progression dilemma: common people that enroll financial use from the government eagerness lose their fuel if they earn likewise than a minuscule unit from faithful sources. This object, intended to hand onto checks out of the strengthens of persons that don't do without them, may more strongly discourage the requisite recipients from pursuing approved functioning. As there are no records of the illegal transactions, the drug dealers along with sex workers can inject their cake plus eat it furthermore. Furthermore, the current structure of social assistance in Canada is such that recipients will lose their income benefits if they begin to earn above $400 per month through legitimate work, leaving this population with limited income generating options beyond resorting to prohibited sources. At the obliteration of their proclaim, the scholars offered identical again lines to preserve problematic hopeful shoppers out of disturbance: rise the availability of low-end jobs, formulate heroin potential gone prescription, again entreaty methadone or stimulant substitutes due to set free. One method of trying to reduce engagement in prohibited income generation among drug user populations with severe addictions is to expand their economic opportunities. This would involve supporting the development of legitimate means of earning income through various low threshold employment opportunities and skill building measures. A recent intervention designed to economically empower drug addicted sex trade workers to develop alternative legitimate sources of income has been shown to have a positive influence on reducing involvement in the sex trade industry. Alternatively, policy makers could intervene by providing addiction prescription and substitution therapies to individuals with markers of serious addiction to decrease their reliance on, and subsequent need to purchase, street drugs. This could be achieved in part through heroin prescription programs and by expanding substitution therapies including methadone maintenance. this is an edited version of guest article on Respectacle by Aaron Rowe
