Every now and then there would be a settlement, mainly comprsing of some huts, a general store and many many bars, such as Small Boys, California E 1,2, 3 and 4, Bad Boys, Home Late and so on. Lots of beer and whiskey being drunk in these parts, so it seemed. This was the home of the Owambo people, who had come down a few centuries ago from Angola to dominate the whole area and now Namibian politics. Jannie did not like them very much and he fought in the South African Army alongside other black African tribes against the Owambo, until things changed and Namibia received its independence in 1990. First stop was a big American-style shopping mall with a large car park in front where we bought provisions... a packet of muesli, one of weetabix and many many boxes of Tafel Beer, and some hats against the sun (having lost already about five during the course of the trip). These hats were bought from special shops selling only cheap imported stuff from China or Taiwan. Incredible. All fake and chealy made, but incredibly cheap. Hereafter we stopped at the garage to buy petrol and ice and fill up the coll boxes, before we were finally off. Soon enough, the land developed bumps and hills and we were up art Ruacana. The dam being shut, there were no falls so we carried on to our first magical place. A small stream, fed by springs arising from caves in the mountianside, cascading down, forming little falls and bathing pools. Way out in the wilds, off road on the rockiest and bumpiest of tracks. Beautiful. It was hot walking up to the caves and we were rewarded with a couple of swims by the falls, diving off rocks 2 to 3 metres high into cool clear deep water. Only a couple of donkeys for company. From there, we were entering Himba territory. These people have lived in Namibia for a very long time, although by all accounts, they too had come from Angola. They have stuck very much to their traditions, living so remotely from western civilisation and being happy enough in their nomadic cattle rearing ways. Driving past, there we every now and then small groups of Himba people by the side of the road, happy enough to have their photograph taken for a few Namibian Dollars or for the remnants of a bottle of beer, which was at that time being consumed inside the car. The light was good and I managed to take some pretty good photos. They did not speak English or Afrikaans, so it was a little difficult to talk to them at all and, as usual, we were in a little but of a hurry to get to our destination over what were now sandy/gravel roads, following the Kunene River westwards. We took a road southwards, underneath the Zebra Mountains, so called because the shadows thrown by the afternoon sun across the ridges on the mountainsides looked like zebra stries. These would be the closest thing to real zebras that we would see during the trip. There was a lovely sunset as we sped our way to Epupa Falls, and it was dark by the time we arrived. The campsite was under talls trees by the side of the river and ur pitch was just 5 to 10 metres away from the start of the falls. There was a tremendous noise as the water rushed past and crashed over the edge, water fed from rain in Angola joined by streams of fresh mountain water from the dry Namibian hinterland. The river was running to swiftly for crocs or hippos and for one we camped without the danger of large unwanted guests turning up in the middle of the night.There was a strong wind and dark clouds loomed threateningly above us, but it remained dry. Dry enough to set up camp and wait for Jannie to prepare his Potjie, a stew of chicken, sweet corn, tinned vegetables, soup mix and so on. Delicious. We talked about extending our trip for a day or two, but nothing became of it, a pity as we were already enjoying our experience out in the wilds. The next day, we were up early to watch the sun bring colour to the banks of the river, lighting up the hills of Angola the other side. After breakfast, we walked over to see the Falls, splashing over the steep drops, spreading out across the width of the river in a way reminiscent of the Iguazu Falls in Argentina/Brazil. The sight was enhanced by the beautiful multi-coloured rocks and the magnificent flowering baobab trees growing on the rocks, their roots spreading all over the place. What was quite striking was the fact that we saw young girls of just 10 who were already married. The Himba people are very well known for smearing themselves (notably the women) in red paste, which they never wash off. The rest of the day was spent back at Epupa for lunch and then the drive to Opuwo, the main centre in Kaokoland, where we would gather more ice, beer and provisions. Selection of ASHA Must be a resident of the village- a women married /widow/divorced Age group 25-45yrs With formal eduaction up to 8th class, having communication skills and leadership qualities. Norm for selection will be one ASHA for 1000 population .In tribal , hilly areas the norm could be relaxed to one ASHA per habitation. At present one lakh ASHA’s have been selected and trained . Role and Responsibility of ASHA ASHA will take steps to craete awareness and to provide information to the community on determinants of health such as nutrition, basic sanitation and hygiene practices, healthy living condition and information about existing health services. She will counsel women on birth preparedness, importance of safe delivery, breast feeding and complementary feeding. Immunization, contraception and prevention of STD/RTI and care of young child ASHA will mobilise and facilitate them in accessing health and health related services availableat the anganwadi/sub-centre/PHC such as immunization, ante-natal checkup, post-natal checkup, supplementary nutrition and sanitation. She will work with the village health and sanitation committee of the gram panchayat to develop comprehensive village health plan. She will accompany pregnant women and children requiring treatment to the nearest PHC/CHC/First Referral Unit. ASHA will provide primary medical care for minor ailments such as diarrhoea, fever, and first aid for minor injuries.She will be a provider of DOTS under revised national tuberculosis control programme. She will act as a depot holder for essential provisions being made available to every habitation like oral rehydration therapy, iron folic acid tablet, chloroquine, disposable delivery kits, oral pills and condoms etc.Adrug kit will be provided to each ASHA. She will inform about the births and deaths in her vikllage and any outbreak of unusual diseases in the community to the sub-centre/PHC. She will promote construction of household toilets under total sanitation campaign. Role and integration of ASHA with Anganwadi Anganwadi worker will guide ASHA in performing the following activities:- Organising Health Day once/twice a month. AWWs and ANMs will act as a resource for the training of of ASHA. IEC activity through display of posters, folk dance etc.to sensitize the beneficiaries on health related issues. Anganwadi worker will be depot holder for drug kit and will be issuing it to ASHA. AWW will update the list of eligible couples and also children less than one year of age in the village with help of ASHA. ASHA will support the AWW in mobilising pregnant and lactating women and infants for nutrition supplement. Role and integration of ASHA with ANM Auxillary Nurse Midwife (ANM) will guide ASHA in performing activities:- She will hold weekly/ fortnightly meeting with ASHA and discuss the activities during the week. AWWs and ANMs will as a resource for the training of of ASHA. ANMs will inform ASHA regarding the date and time of outreach sesion. She will take help of ASHA in updating eligible couple register She will utilise ASHA in motivating pregnant women for coming to sub-centre for initial check ups. ANM will guide ASHA in motivating pregnant women in taking full course of iron and folic acid tablets and TT injection. ANM will orient ASHA on the dose schedule and side effects of oral pills. ANMs will educate ASHA on danger signs of pregnancy and labour so that she can timely identify and help beneficiary in getting further treatment. ANMs willinform ASHA on date, time and place for initial and periodic training schedule. Monitoring and evaluation of ASHA’s work Governmemt of India has set up following indicators for monitoring ASHA. 1. Process Indicators Number of ASHAs selected by due process Number of ASHAs trained % of ASHAs attending review meeting after one year 2. Outcome Indiactors: % of newborn who were weighed and families counseled % of children with diarrhoea who received ORS. % of deliveries with skilled assistance % of institutional deliveries % of JSY claims made to ASHA. % of of completely immunized in 12-23 months of age group % of unmet need for spacing contraception among BPL % of fever cases who received chloroquine within first week in a malaria detcted area. 3. Impact Indicators: IMR Child malnutritionrates Number of cases of TB /Leprosy detected as compared to previous year. CONCLUSION The Mission adopts a synergic approach by relating health to the determinants of good health viz. of nutrition, sanitation, hygiene and safe drinking water. Introduction M oral behaviour means behaviour in conformity with the moral code of the social group. `Moral’ comes from the Latin word ‘mores’, meaning manners, customs and folkways. Moral behaviour is controlled by moral concepts. Moral development has both an intellectual and an impulsive aspect .Children must learn what is right and what is wrong. Then, as soon as they are old enough, they must be given explanations of why this is right and what is wrong. Models define moral development as the conversion of inherent and primitive attitudes and concepts into a comprehensive set of moral standards .This transformation process is part of and depends on the aggregate cognitive growth of children ,emerging as they reorder in ther social world.Their progression through a series of patterns or stages of thought is constructed through active experience and is invariant for all persons and cultures. Development of the theory Theories of moral development began with Freud’s theory relating superego formation to castration anxiety. Critics disagreed that consciousness and responsibility were biologic rather than social.Piaget’s view was that justice was the core of morality.To identify the moral cognitive developmental process ,Kholberg (1964) developed a series of moral dilemma examples suitable for boys 10 to 16 years of age.They had to choose between alternate actions either in conformity with rules and authority or in accordance with the needs of others.Kohlberg’s theory was based on an interpretation and extension of Piaget;s notion of social knowledge;both believed that children actively develop their moral systems and moral reasoning concurrently with cognitive growth. Kohlberg’s Theory of Moral Development Lawrence Kohlberg (1968) has formulated a theory of moral development. Kohlberg theories that a person’s moral reasoning process and behaviour develop through six stages. Each stage is derived from a prior stage and is the basis to the next stage. He defined three levels of moral development. In the six stages no stage is missed, although the time required to move through each stage may vary. Few people progress through all six stages.Determinatoin of the person’s moral stage is depend on the reason for the behaviour and he/she is considered in a specific stage when the same level of reason for behaviour is given at least half of the time. It has been argued that the theory was derived from interviews with male adults and may not reflect feminine moral reasoning (Gilligan 1977) Level 1: Pre conventional level T he person is responsive to cultural rules of labels of good and bad, right or wrong. Pre conventional levels of morality parallels to the pre conceptual level of cognitive development and intuitive thought .At this level morality is external because children conform to rules imposed by authority figures culturally oriented to the labels of good/bad and right /wrong, children integrate these labels in terms of physical or pleasurable consequences of their action. Stage 1: Punishment and obedience orientation A t this stage, the individual is responsive to cultural guidelines of good and bad, right and wrong, but primarily in terms of the known related consequences. Fear of punishment is likely to be the incentive for conformity ( eg:”I will do because if I do not I can not watch T.V for one week”) Average age toddler to seven years. Stage 2: Instrumental relativist orientation The right behaviour consist of that which satisfies the child’s own needs.Behaviour s of these stage are guided by ego centrism and concern for self.ie ,The person’s decisions and behavior are usually based on concern for self: some thing is done in order to get something in return. Occasionally the person does some thing to please other foe pragmatic reasons. There is no feeling of justice, loyalty or gratitude. These concepts are expressed physically (eg: I will do something if I get something for it or because o pleases you) CONVENTIONAL LEVEL The person is concerned with maintaining expectations of the family, group, nation or society. A sense of guilt has developed and affects behaviour. The person values conformity. Loyalty and active maintenance of social order and control. Conformity means good behaviour or what pleases or helps another and is approved. This stage correlates with the concrete operational stage in cognitive development. Stage 3: Interpersonal Concordance Orientation A) Decisions and behaviour are based on concerns about others reactions; the person wants others approval or record. The person has moved from egocentricity to consideration of others as a basis for behaviour. Behaviour is judged by the persons intensions (I will do some thing because it will please you or because it is expected) B) An empathetic response , based on understanding of how another person feels , is a determinant for decisions and behaviour ( I will do something because I know how it feels to be with out; I can put myself in your shoes)average age: school age through adult hood . Most American women are found to be in his stage. Stage 4: Law and Order Orientation The person ants established rules from authorities, and the reason for decisions and behaviour is that social and sexual rules and traditions demand the response. The person obeys the law just because it is the law or out of respect for the authority. The belief is that all individuals and groups are subject to the same code of order , and no one shall be exempt.( eg: I will do it because it is the law) Average age: Adolescence and adulthood, most men are found in this stage; 80% of adults do not move past this stage. Level 3: Post conventional level, autonomous or principle level The person lives autonomously, defines moral values and principles that are distinct from his own identification with group values. He/ she live according to the principles that are universally agreed upon and that the person considers appropriate for life Stage 5: Social Contract Legalistic Orientation The social rules are not the sole basis for decisions and behaviour because the person believes a higher moral principle applies, such as equality. Justice or due process. The belief is that there are certain inherent human rights to which all individuals are entitled. Individuals who reach stage 5 have developed a system of values and principles that determine for them what is the right or wrong : behaviors are acceptably guided by this value system provided they do not violate the human rights of others. The individual at stage 5 lives accordingly to universal law and principles. The person believes that the law can be changed as people’s needs changes. Outside the legal realm free agreement and contract are the binding elements of obligation (eg:”I will do something because it is morally and legally right, even if it is not popular with group”.) Average age: middle age or older adults. Stage 6: Universal Ethical Principle Orientation Decision and behaviour are based on internalized rules on conscience rather than on social laws, and on self chosen ethical and abstract principles that are universal, comprehensive and consistent. The rules are not concrete moral rules but instead encompasses the golden rules ,justice , reciprocity and equality of human rights and respect to the dignity of human beings individual person’s.Humam life is inviolable. The person believes there is a higher order than social order has a clear concept of civil disobedience and will use self as an example to right a wrong. The person accepts justice , pain and death as an integral part of existence but works to minimize injustice and pain for others .(e.g. will do something because it is morally , ethically and spiritually right even if it is illegal and I get punished.) Average age: middle age to older adult. Conclusion . Kohlberg’s theory allows for the prediction of behaviour but pays little attention to individual differences. Children can learn to behave in a socially approved manner through trial and error through direct teaching or through identification. References 1.
