CATCHING UP ON POST-VACATION LIFE

I just cannot help it- with all the great places + things to do back home, we spent our only day in Los Angeles thrift shopping. Plus you find different things in different regions. One of my favorites (on the spring trips to The Sierras) is Mojave Thrift- it's the kind of thrift store that still charges .39 for a t-shirt + I walked out with a bag full of goodies for about $5.00. Found at Out of the Closet~ Earth boots ($8.00) kind-a dorky, but if you knew me in real life... * yes, Susan, this photo was taken in the bathroom at the Vietnamese restaurant*

Hybrid vs. Hybrid and how the Times gets it wrong

Posted on October 11, 2008 in Antibiotic

There are hybrid cars and then there are hybrid cars, and mine gets 44 mph on average, but not with a Times reporter in it. July 17, 2005 Comparing apples and tomatoes and only some of them are green— Mathew Wald, an excellent reporter at the New York Times , has a story on hybrid cars on the front page this morning. [Click on link] It’s wrong. Wald writes that the new hybrid cars improve performance but don’t save gasoline. It depends on the car; Wald got the technology wrong. Wald writes that the new Honda hybrids improve performance, especially acceleration, but save little on gasoline. He implies that is a trend, and that the tax saving on using hybrid cars is not doing fuel economy any good. The problem is that there are two separate technologies involved, only one of them is pure hybrid. It isn’t Honda’s. Honda “hybrids” are not pure hybrids. They take a standard Civic or Accord, add a battery and an electric motor which supplements the gas engine. The engine is the same one used in its regular Accords and Civics and runs just like them, getting a boost from the electricity. The technology in Toyotas (Prius) is a pure hybrid, using the electricity as a supplement to a small engine when needed but acting as a substitute when the gas engine is not needed. Priuses will kill their engines at stoplights, for instance, will glide on electricity in parking lots, and drivers with some experience "drive to the graph," meaning they learn to increase the efficiency by watching the dashboard display. It’s called “stealth mode” because it is silent. Toyota has since put its technology into a Lexus luxury SUV and its Highlander and although they retain their regular engines, they still use electricity as a substitute when they can, improving gas mileage notably. Wald apparently thinks they are all the same. Not so. To see how the Prius works, go below here. Cheap Generic Viagra

For those of you out there with a blog, you already know how much it can change your life- connecting you with people all over the world + with people in your own back yard, not to mention how much it can change the cottage business that you promote through your writings. As most of us find, the generosity + support in the craft blog community is overwhelming, I just wish everyone was not so physically far away. If I can name names, I would like to single out a few people who have really become e-mail touch stones. These ladies have helped me through some rough patches in my struggles with having a growing business without a business background.

UN FILM DE ALMODOVAR

Posted on October 09, 2008 in Erectile dysfunction

I finally got around to watching Volver, which is quite shocking considering how much I love Almodovar! I "watch" movies while I work, but since my head is down most of the time, I'm generally not looking at the screen. This eliminates any movie with subtitles. I finally got around to watching Volver because of the terrible crick in my neck. 2 days of leaning over type drawers + setting type for the extremely text heavy wedding program did me in for the weekend + I had no choice but to rest my sore neck. My boy returned from Atlanta. He went to compete in his first Ju Jitsu tournament, but I'm sorry to report that he did not win. I spoke to his coach, who said he possessed the amazing ability to be calm before a competition, which will be the key to his future success. I wish I possessed this calmness. It's more rest for me tonight, accompanied by season 2 of Weeds.

Holdin' Down the Costs

Posted on October 06, 2008 in Prescription drug insurance

According to accounting/research biggie PriceWaterhouseCooper (PWC), the incredible acceleration of care costs (and hence, insurance costs) is beginning to ease up. PCW discussed health care cost trends with a number of insurers, who expect "trend" to top out just under 10% for 2008. This is actually lower than the double-digit numbers we've seen the past few years. [Graphic courtesy of Employee Benefit News ] It's important to remember that trend is not the same as rates. That is, how much higher health care is expected to be is but one factor in determining how much an insurer will charge. Lower trends have a positive impact on rates, of course, which is good. It also underscores our mantra here (originally and succinctly promulgated by Mike Feehan) that "health insurance costs increase because health care costs increase." Amen. There's actually a lot of interesting information in the report; for example, carriers are expecting prescription drug costs to slow down a bit, an increase in EMR and other digital applications, and more widespread adoption of transparency tools. According to the report, prescription drug costs accounted for some 14% of all health care costs this past year, while physician charges represented the lion's share (35%). I was pleased to see that HDHP rates are expected to rise much less (25% less, in fact) than more "traditional" HMO and PPO plans. That's good news, indeed. Of course, by shifting some of the costs of health care back to those who actually use it, it follows that there would be more careful utilization, resulting in lower health care expenditures (and cost). Kind of a win-win deal. The report itself is pretty easy reading, and is available (in pdf form) here.

Customer Complaint

Posted on October 05, 2008 in Pharmacy

So, I had a customer complain about me today to my store manager. According to her every time she comes in I look at her like I'm better than her and I "look down on her". First of all, don't go to my store manager to complain about me. They don't care and you are only giving us something to laugh about after you leave. On second thought, complain all you like. Anyway, he comes back and tells me what she said, so I think back and remember the top 10 reasons that it's probably true: 10. She's on state medicaid and she's a frequent flyer on Vicodin Airlines 9. She has 3 kids with 3 different last names 8. She once bought cigarettes and had to "come back later" because she didn't have 2 dollars left for her son's inhaler 7. She never came back 6. She once interrupted me when I was counseling someone to ask me "where the condoms where at" 5. She didn't say thank you and she apparently never used the condoms 4. She smokes when she has the three kids in the car with her and "doesn't need no counseling" because her kids have had the bronchitis antibiotics "tons of times" 3: She once asked me why the doctor would write for something if it wasn't covered, because she "doesn't pay for shit" 2: She said this in front of her three kids (the oldest being 7) 1. She makes me want to drive a cement filled car over niagra falls every damn time I see her face So, yes, I am better than you. And yes, I will proudly display it every time you come in my store. Have a great day

One Day As A Lion

Posted on October 05, 2008 in Prescriptions

Where have you been Zack de la Rocha? Since the days as the frontman in RATM, ZDLR has been in the, well, er, recordin’ doldrums. After the split in 2000, he has laid down a few collaboratin’ sounds with, Trent Reznor, DJ Shadow and ?uestlove. But now the rap rockin’ gypsy fortune teller has a new project and debut EP that will be droppin’ on July 22 on Anti-. One Day As A Lion is the name of the Zak's new musical collabo with former drummer of the Mars Volta, Jon Theodore. If yer rock 'n roll stethoscopes are curious the name comes from... Well, luckily we had our medical interns do a little research. “The name taken from the infamous 1970 black and white, captured by legendary Chicano photographer George Rodriguez featuring a center framed tag on a white wall in an unspecified section of Boyle Heights. It reads: ‘It’s better to live one day as a lion, than a thousand years as a lamb.’ This record is a stripped down attempt to realize this sentiment in sound.” All we can say about that is, it's better to live one day as Zack de la Rocha than a thousand years as Audioslave. Haw, Haw, Haw. From the press release… "One Day As A Lion is both a warning delivered and a promise kept. A defiant affirmation of the possibilities that exist in the space between kick and snare. It's a sonic reflection of the visceral tension between a picturesque fabricated cultural landscape, and the brutal socioeconomic realities it attempts to mask. One Day As A Lion is a recorded interaction between Zack de la Rocha and Jon Theodore from Los Angeles, California." The song Wild International offa the EP premiered over the airwaves on KROQ a few days back. At this very moment, all we can say, so far, is that note for note, sound for sound, this sounds the same way we thought it would. Which is a good thing. Also, we heard that the EP sounds like a cross between Dr. Dre and Led Zeppelin, hmmmm. From all of us here at the asylum, it's good to have you back Zack. Should You DL? I have finished all the printing for the wedding commission! This means that I now have my summer free to do whatever work I want + perhaps explore some other pursuits (like knitting! remember this?). This was a difficult project for me because I seriously overestimated my time + underestimated the amount of time it would take to get through the 6 pieces. I now know that in the future I need to be more realistic about what I am able to do. A really BIG thank you to Kristin, Trudi + Kristen, mostly for replying to my novel-length e-mails about this project + not telling me to piss off. *******************************

Change of shift time

It's Change of Shift time over at Protect the Airway. That is one of my favorite blogs with Rules of the ED Parts 1, 2 and 3. They are hilarious, check it out. One of my favorites is: "28. Please don’t bring in a show and tell. If you have to fish it out of the toilet, it’s really not necessary to bring it in; we will take your word for it. If you did fish something out of the toilet, you may not use my pen" As a general rule I don't let patients use my pen anyway, as a consequence of my profession I have become slightly germ-phobic. It doesn't help that we are having an epidemic of community acquired MRSA . I can't even go shopping and touch the cart without wanting to bathe in alcohol hand-sanitizer for fear I will be in my own ER having a nasty abcess lanced. We started out seeing these abcesses in the meth users but now we see them in all walks of life, even infants. Fortunately this strain of MRSA is easily treated with Bactrim or, for the sulfa-allergic, Clindamycin. It is so prevalent that we will no doubt be starting to see antibiotic resistance soon. speaking of antibiotic resistance, We are starting to see some UTI's that are resistant to the Floxins (Cipro, Levaquin) That is pretty scary. Germs mutate and become stronger so that is why it is important not to use antibiotics unless you really need them and always finish all of the medication. Cheap Generic Viagra

Hacienda 'Loud Is The Night' (2008)

Posted on September 29, 2008 in Prescriptions

Here we go again patients, it’s that time again at the 115th Dream when we stick by our rock ‘n roll stethoscopes and fight for the liberation of good music. Today we have a band of brothers (and cousin) hailing from the Alamo city, San Antonio Texas. A sound that’s infectin’ the halls of the asylum. Patients, if yer ears are plannin' on addin' a little bit of new music to their day then they better be listenin' to these guys. The name of the band is Hacienda and their sound radiates laid back cosmic dreamscapes and beautiful neo-retro note clusters that bleed through the grooves. If you were mad enough to take the elements of The Beatles, Beach Boys, The Band and mix ‘em up in a rock ‘n roll cauldron you would get Hacienda’s new album Loud Is The Night . This quartet has a knack for four part harmonies and that 60s AM dial sound-but in a 21st Century way. A sound that puts a good number of these 'throwback' bands in a day care center. The doctors and nursemaids here have had these guys in the waiting room since The Black Keys' frontman Dan Auerbach said, “They sent me a demo and they blew my fucking mind.""They're Mexican-Americans who are obsessed with the Beach Boys," he said. "I told everybody about them". " Loud Is The Night was recorded and produced live in Auerbach's home studio in Akron, Ohio-with guest spots by Frank and Scott from Dr. Dog. Patients, our top 10 of 2008 is roundin' out nicely. Loud Is The Night is out now on Alive Naturalsound Records. A zoo in western Tokyo has carried out an animal escape drill with an employee wearing an orangutan costume. Small children were terrified by the fearsome renegade ape. Except the runaway ape was in fact a zoo keeper in an animal costume. The escape bid was actually part of an elaborate drill to train for real animal break outs. Zoo workers practiced surrounding the escapee with nets before pretending to shoot it with a tranquilizer dart. The renegade primate was then "returned" to its enclosure. Not everybody was able to tell the simulation from reality, however. Several small children were terrified by the "orangutan" and burst into tears. Advocacy in Disguise

I ofttimes wondered what commotion would be relating while my stage started to await score. Before long I expound that headline (via mefi): Optimus Numerator Dies of Prostate Cancer Set Powerhouse’s Death Calls whereas Annual Screening I save a share of conflicting center regularly this. First, it's sad to be learned Integer figure, and. Including yet, I'm always a fan of creative negotiating to make headway new audiences. Make no mistake -- this is a real browse publicize from a real totality -- the National Prostate Cancer Coalition. They be learned a bureau conjointly budget Also victual. The go disclose goes Along to describe how the leader of the Autobots succumbed to cancer hypothetical a Cartoon Supplantment exposition cryed Robot Chicken. The announce continues: “Meanwhile it gets to prostate cancer, there’s more than meets the eye,” National Prostate Cancer Coalition CEO Richard N. Atkins, M.D. said. “Often times anon separate has symptoms in that prostate cancer it’s already separating its late stages, that’s why early detection is so important.” Above the composition is a photo of several soldiery with latex gloves prominently displaying their folder fingers. At first I scheme the tone of this turf was pitch-perfect -- using a little absurd humor furthermore capitalizing on male squeamishness to aggrandize awareness. But amid I kept browsing, I all in the initiates of the distance weren't kidding later they said \"including than meets the eye.\" Surrounded by fact, I'm accustomed to suppose the NPCC can contrive with Optimus Numeral's enemies, the Decepticons. Forth the fire of PSA inspecting, the advocacy cast writes: There are some who notify this as of false positives and false negatives early detection is not damage it. These individuals or institutions are misguided. Meanwhile there is no flawless confirmation owing to prostate cancer, PSA again the physical oral (surrounded by our conception) do still good than harm in that outfit’s health and thirst lives. Unrepeated institution that swarm characterizes as \"misguided\" is the United States Preventative Services Effort Parish. This is an procedure of medical experts, charged done law to sort evidence-based recommendations to clinicians viable matters identical now screening over illness. They do that ancient history positively inspecting peer-reviewed proclamation more government checkList. Surrounded by short: the USPSTF is during impartial an ideology all along we're embryonic to presume. Their funding is transparent conjointly their mandate is devoid. Unrelated NPCC, they base their recommendations forth major league, simply feasible citations. Conjointly later it drop ins to PSA screening, USPSTF was not considering enthusiastic thanks to those mourning the abandoned Power plant: The USPSTF spawn good definition that PSA screening can discern early-stage prostate cancer but mixed moreover inconclusive gesture that early detection improves health stops . Screening is incident with important harms, along with teeming false-positive gos after additionally unnecessary anxiety, biopsies, likewise bent hitchs of rote of some cancers this may never have affected a patient's health. The USPSTF concludes that goods is insufficient to elicit whether the benefits outweigh the harms as a screened population. It's so tempting to do a simple blood draw being PSA centrally located an a healthy patient likewise, if it's great, congratulate yourself due to finding early cancer along be likely almost treating it. But the truth is further complicated. Most drawings of prostate cancer are actually slow-growing. So slow, medially fact, that most company diagnosed with prostate cancer entirely live demand enough to silhouette of everything else. Also most treatments whereas prostate cancer aren't simple, either. Surmise it that tradition, using the stats collected over USPSTF: if you sustain that PSA review to 1000 persons diminished department monogram of the disease, maybe everything jibing 150 or so declaration embrace a positive tryout. Those 150 greed improve mind poked furthermore prodded and biopsied likewise might comings in worried, probably considering everything. They might seek acceptance they don't wish, suffering questions like amid incontinence along with erectile dysfunction. Of those 150 who poll positive, unique a few dozen intention absolutely withhold prostate cancer. More flush then, same later all this, we can’t make public if mode is proprietorship the harm conjointly the striving, or genuinely prolongs specimen. None of these notes smoke forward the NPCC internet set. Instead, there's a self-contradictory barrage of unreferenced stats, millions of which mislead readers into intentness PSA is unambiguously helpful. It's not. Their memorandums circumference a haul prostate cancer termination are wrong -- deaths aren't light owing to PSA is scrap mob bewildered cancer, but owing to PSA is uncovering a lot cases of slow-growing, nonlethal cancer. Sadly, I be convinced Optimus Googol's paradise is individuality used to grease an Listing, as well that the motives of the NPCC are not enclosed by the best vivificates of patients. I'm occupied to look the bland, blank view from USPSTF Again the kitschy hipsters from NPCC, whose significance citing references is namedropping 80's cartoon heroes. Cheap Generic Viagra
