Big Pharma: Everyone's Favourite Market Failure

Posted on October 06, 2008 in Erectile dysfunction drugs

Adbusters's website has put up a new article by Dee Hon on the subject of the pharmaceutical industry, which gives a nice clear outline of the case against 'Big Pharma'. Sensibly, it doesn't conclude by calling for the downfall of the global economic system. Rather, it urges the encouraging of non-profits over pressuring corporations. Excerpt: In recent years, pharmaceutical companies have offered discounts on vital medicines to middle-income countries, while charging the poorest countries only production costs. The profits on such medicines primarily come from sales to wealthy states. Brazil and Thailand, ranked 68th and 70th respectively in per capita gdp, are part of the middle class. Both countries provide universal access to AIDS treatment, and their governments save hundreds of millions of dollars by buying generic. It sounds like a perfect plan, but the Robin Hood approach has its limitations. Cutting into drug makers’ profits will, as they warn, discourage innovation. Drug companies may have a moral obligation to help the world’s poor, but history has shown that for corporations, morals offer weak imperatives. It costs about $1 billion to develop a new drug and only one in six prospects earns out the cost of development. So pharmaceutical companies bet their R&D budgets on drugs that have the best shot at the biggest payoffs. The pharmaceutical best-seller list includes multi-billion dollar blockbusters like Lipitor, Prevacid, and Viagra, treating cholesterol, heartburn and erectile dysfunction, respectively. They’re the disorders of the wealthy, aging and overfed West. Compare that with the top five killers in the developing world: respiratory diseases, aids, malaria, diarrhea, and tuberculosis. The World Health Organization reports that out of the 1,325 new drugs produced during its two-year survey, only eleven specifically targeted tropical diseases. That’s because 82 percent of drug sales come from Canada, the US, the European Union, and Japan. Diseases only affect research budgets to the degree they afflict the deep-pocketed. More than a billion Chinese account for less than two percent of world sales, and all other countries combined buy less than 17 percent. Cheap Generic Viagra

Definitions of 'euthanasia'

Posted on September 29, 2008 in Generic medical release

With so much talk about 'euthanasia' these days after the Terri Schiavo affair (March 2005), and the affirming decision of the US Supreme Court (January 2006) in the Oregon physician-assisted suicide case, it seems time -- in the interests of mutual understanding -- to define the principal words being used. Thus............. ASSISTED SUICIDE: Helping a person to end his or her life by request in order to end suffering.. (Rarely prosecuted and only lawful in Switzerland where the reasons must be altruistic.) PHYSICIAN-ASSISTED SUICIDE: Medical doctor helping patient to die by prescribing a lethal overdose. Patient can chose whether to drink it. (Lawful only in Oregon, Switzerland, Netherlands and Belgium.) EUTHANASIA: A broad, generic term meaning 'help with a good death.' VOLUNTARY EUTHANASIA: Death by lethal injection by doctor when requested by patient. (Only lawful in Belgium and the Netherlands for the terminally or hopelessly ill.) NON-VOLUNTARY EUTHANASIA: Using powerful drugs, doctor ends life of suffering, dying patient who is comatose. Illegal, but happens all the time, discreetly, in the interest of compassion. TERMINAL SEDATION: Upon patient request, doctor puts patient into deep sleep with medications, during which time the patient dies either of the underlying illness or starvation/dehydration. Widely practiced and generally accepted as ethical and lawful. MERCY KILLING: Taking the life of another person in the belief that this is a compassionate act because the ill person is unable to do so. Unlawful. Apple's inner heat keeps cities afloat. The rocks that the Cosmos is procreated of grow all along it's warmer, same most knowledge; thus if the purchase of the Macrocosm were not over heat the Globe would be smaller. The Elephant Has Landed

Posted on September 26, 2008 in Medical care

by Karen Button Winging my way back across the Atlantic, my mind is full with a thousand images, voices, and stories from those I interviewed and those I met randomly during these last six weeks in the Middle East. My last night in Jordan, unwilling to waste time sleeping, I visited with friends, schemed how additional humanitarian aide could be funneled into war-torn Iraq, and conducted one last interview, this one with a doctor who’d just returned from visiting the health clinic he once directed, but that has been in shambles since US troops shot it up in November. He shows me pictures from his visit: a blackened room where the maternity ward once was, a gaping hole in the ceiling of a treatment room where a missile ripped through, an outside wall strafed with bullet holes and surrounded by barbed wire has a “3DB” spray-painted in black just under the health clinic’s sign. “What’s that?” I ask. “It means three dead bodies,” he replies impassively, as he flips through images. “They spray-paint codes on the sides of buildings after they’ve raided them,” he says of the troops. In another photo, a women stands atop a heap of rubble that was once her house. He doesn’t know what the “BG80” sprayed across a surviving slab of concrete means. I hope it doesn’t refer to 80 dead, but given the hundreds killed, I know that it could. I think back to a conversation I had with Nermin, a 23-year veteran journalist from Baghdad, while we were both in Turkey. She was telling me of the countless times she’d stopped in Fallujah on her way back home from somewhere. Fallujah, famous for its kebobs, was the perfect mid-way stop for a bite to eat. Last November, Nermin went into Fallujah knowing it had been devastated but not prepared for how extreme the devastation was. A trip that was normally 45 minutes now took her five hours. The kebob stand was, of course, gone. Her friend from the Iraqi Red Crescent who’d gotten her in was staying in Shurta, a neighborhood, the friend said, that wasn’t destroyed like other areas. But, it was, Nermin told me, every building either flattened or full of bullet holes. “I’ll never forget the first house I saw. There were beautiful green curtains in a second story window blowing gently in the wind. The main gate was open and in the garden a small bike, as if someone were coming home. But beyond that sat a car, completely destroyed. “I began to think all my dreams were in that bedroom. And where were the owners…were they alive or were they dead?” She looks off into the distance. I follow her gaze, as if I could also see these billowing curtains whose color I imagined as the green of a tree fully leafed out, a color I’ve always thought of as the color of life. “Fallujah was called ‘The City of Minarets,’” she continues, bringing me back. “But now there is no call to prayer. Being a Muslim you are called five times a day, but there was only silence. “I carry a phone book that was given to me in 2003. Fifty of my friends who are in that book are now lost. For the Americans, every Iraqi is a terrorist until they prove it, not deny it.” As I step into the clean, well-organized and climate-controlled airport I wonder how many Americans could hear something like that, I mean, really listen. Most, it seems, prefer their news as sanitized as the airport. Waiting out plane delays due to bad weather, I watch with amused detachment as CNN delivers their version of domestic and international events. I have that very surreal feeling we’ve all had when no one wants to talk about the elephant that’s clearly sitting in the middle of the living room. Listening to Karl Rove being described as the next “Deep Throat” is a clear indication I’m back in the States. As for Iraq, hardly a word is mentioned until a suicide bomber, who’s targeted American troops giving out candy, kills a number of small children. As horrible as this is, the stations play it out as if it’s the only news from Iraq, as if US troops aren’t also killing small children. As a friend later tells me after reading my report about US attacks on Western Iraq’s hospitals, “I know this kind of stuff is happening, but I don’t want to believe it.” I agree with him, it’s painful to look at what your country is capable of. And it’s much easier to turn away from it if we’re not reminded of it each night when we turn on the news, which is why they don’t show us. But, it is happening. Right now. As I write this. As you read it. Now, what will we do… now that we know?

Rethinking Proactive Myeloma Treatment

Posted on September 24, 2008 in Erectile dysfunction treatment

My myeloma is still in a early stage, barely even smoldering. Thankfully, it has not grown rapidly; it seems to double about every 18 months. Nevertheless, I have asked my doctor to be very proactive in two ways: (1) Testing for potential problems, and (2) Treating the disease before symptoms develop. Although studies have shown that early treatment does not result in longer survival, my theory has been that early treatment should at least yield a higher quality of life by delaying symptoms like broken bones and failed kidneys. Mayo clinic, on the other hand, has recently published a new consensus statement outlining a treatment algorithm for newly-diagnosed myeloma patients titled " Treatment of Newly Diagnosed Multiple Myeloma Based on Mayo Stratification of Myeloma and Risk-Adapted Therapy (mSMART" . The abstract is here and the full text is here. It divides newly-diagnosed patients into two groups, high-risk and standard risk, and further divides both of those groups again into a class with active (symptomatic) myeloma and another with smoldering (non-symptomatic myeloma). Lucky for me, I seem to be in the standard-risk group with smoldering myeloma. For this group, Mayo's algorithm suggests NO treatment. If I had started my doctoring at Mayo Clinic, I would very likely never have taken thalidomide, unless as a participant in a clinical trial. I don't know if that would have been good or bad; I took the thalidomide with my eyes wide open and was glad that my doctor was treating me aggressively. But now I'm thinking I'll ask him to be a little more conservative in treatment. Here are some reasons: All treatments have side effects. For example, with thalidomide I had rash, low heart rate, erectile dysfunction, slow bowel, weight gain, and possibly a minor deep-vein thrombosis and peripheral neuropathy. Happily, none of those were show-stoppers, and all but the rash are gone now. However, the next step for me is Revlimid with dexamethasone, which could easily cause more-serious side effects. To some extent, each treatment may be thought of as an arrow in the quiver. Once it's been shot, it's gone. Thalidomide seemed to work at first, but not any more, and it will most likely be unavailable later when I might need it more. Treatments can cause the myeloma to mutate and become more aggressive. I suppose this is why early treatment doesn't actually extend survival. Since my myeloma is progressing slowly now, maybe I'm better off not provoking it unnecessarily. Treatments can cause other cancers. I think that applies mostly to the older, standard chemotherapy treatments like melphalan, but those treatments may be all that remain for me if I use up the other arrows in the quiver too soon. Treatments can even cause the very symptoms that we are most trying to avoid from the myeloma. I know a man whose kidneys are failing because of treatments, not because of the myeloma. I'm quite content to try curcumin next. It's an unconventional treatment, to be sure, but there is good reason to believe that it might work, more than speculation. Further, it does not seem to carry the risks inherent in most of the prescription treatments. I'm taking two months of NO treatment after discontinuing thalidomide, then getting tests done June 26, for the final report card on the thalidomide. I will start the curcumin later that same day. I am very interested in opinions of anyone else who reads this. If you see an error in my facts or my thought process, or even if you agree, I would value your opinion. I'm thinking about my life here. Thanks! Sunshine and Sweet Pea were out a few nights ago. SOMEBODY'S MOTHER

Posted on August 21, 2008 in Erectile dysfunction drugs

The was old and ragged and gray, And bent with the chill of the winter's day; The street was wet with a recent snow, And the woman's feet were aged and slow. She stood at the crossing, and waited long, Alone, uncared for amid the throng Of human beings who pass'd her by, Nor heeded the glance of her anxious eye. Down the street with laughter and shout, Glad in the freedom of "school let out," Came the boys like a flock of sheep, Hailing the snow piled white and deep. Past the woman so old and gray Hasten'd the children on their way, Nor offer'd a helping hand to her So meek, so timid, afraid to stir, Lest the carriage wheels or the horse's feet, Should crowd her down in the slippery street. At last came one of the merry troop, The gayest laddie in all the group; He paused beside her, and whispered low, "I'll help you across if you wish to go." Her aged hand on his strong young arm She place, and so, without hurt or harm, He guided the trembling feet along, Proud that his own were firm and strong. Then back again to his friends he went, His young heart happy and well content. "She's somebody's Mother, boys, you know, For all she's aged and poor and slow; And I hope some fellow will lend a hand To help my Mother, you understand, If ever she's poor and old and gray, When her own dear boy is far away." And "somebody's mother" bow'd low her head In her home that night, and the prayer she said Was: "God be kind to the noble boy, Who is somebody's son and pride and joy!" by Mary Dow Brine

My Winding Road

Posted on August 19, 2008 in Erectile dysfunction drugs

There is a long and winding road High in the wooded hills, Where deer graze silently in the trees And a tinkling waterfall spills. Nature's symphony, the only sound Whispering softly in the breeze, Embracing me oh so gently Caressing the tall, majestic trees. Small pebbles on the dusty road Crunch softly as I stroll, A spotted fawn watches trustingly, From a fern enshrouded knoll. The scent of wild blackberries, Drifts through this woodsy place, Buzzing bee's polinating blossoms With accuracy and grace. The peace and serenity in this place, Quicken deep within my soul, To the youthful years long gone past When I once walked a winding road.

The above link is to a very interesting argument (article and comments) about the Schiavo case from the perspective of disability: rights and treatment of persons with disabilities remains difficult for the non-disabled to really grasp in anything like a systematic fashion because so few take the time to think about or grapple with these issues in any depth. However, it must be noted that, perhaps more so than in most "communities of interest," there is a great diversity of opinions and issues among those people who have or who deal with disabilities. There are some commonalities -- and concern about euthanasia/genetic testing/abortion is one of them -- but I'm deeply suspicious of anyone who says that the Schiavo case is "simple" if taken from the "right" perspective....

the Lonesome Death of Otillie Lundgren

Posted on August 09, 2008 in Generic biologicals

Americans have no memory. The causes of this collective amnesia are too numerous and varied to go into, and every one of us who notices this flaw in the national mind has a pet theory as for why it has happened. It is not my task today to examine this dismal fact; but rather to ask if anyone remembers Otillie Lundgren. The circumstances of her death were bizarre but not unique to her time. She was 94, and she died after receiving mail tainted by anthrax. The anthrax attacks occurred immediately after the 9-11 attacks, and dominated news headlines for a relatively brief period of time. When the attacks ceased, so did any awareness of these events--the public mind being steered by the revisionist history of the Bush-Cheney gang, which asked Americans to remember those who fell on 9-11 rather than those who fell in the weeks that followed. Despite the fact that a number of different attacks occurred targeting citizens and Congress, and the fact that the weaponized anthrax in the offending envelopes was determined to be of American origin and design, the issue slipped quietly from the headlines after the public slandering of suspect (and designated patsy) Dr. Stephen Hatfill was completed. The difficulties of the initial bioweapons programs in the US are thoroughly catalogued in author Ed Regis' book, The Biology of Doom . Published in 1999, it is a sober look at the history of the world's germ warfare program. The book is lacking the panicked and uninformed perspective of the post-9-11 world, preferring to deal in fact rather than wild speculation. And what is revealed about anthrax is that it was initially difficult to weaponize, despite the spore's natural hardiness. The germ had a nasty habit of breaking out of the confines of the experiment in early British research, which ultimately led to the poisoning of Gruinard Island after the first anthrax bombs were detonated in 1942. Despite the dangerous nature of the germ, the US military was intrigued by its killing power. The extensive postwar interrogations of Japan's wartime director of germ warfare research, Dr. Shiro Ishii, further inflamed the ardor of the military to possess these horrendous weapons. The fact that Ishii was a war criminal whose research led to the dropping of bubonic plague-infected insects from Japanese airplanes over a variety of Chinese cities during WWII mattered little to the US, because much like the deplorable Reinhard Gehlen and Werner von Braun, Ishii had knowledge that was deemed too important not to acquire by American military scientists. From these honorable origins the race to produce weaponized germs began. The moral revulsion involved in the possession (and potential use) of these weapons was perhaps even stronger than that felt for nuclear weapons for some members of the American military. But many felt justified in the production and research of such horrors. Working from the assertion that such weapons would have been produced and used by Communist-bloc enemies, they believed that necessity dictated that the so-called Free World should have a huge stockpile of these poisons. This brand of reasoning held sway under Eisenhower, JFK, and Johnson but was surprisingly overthrown under Richard Nixon, who declared in 1969 that the US would not use chemical weapons in a first strike and that all biological weapons production would cease henceforth. An accident in Utah that resulted in the death of thousands of sheep from nerve gas was the prime mover behind the Nixonian renunciation rather than any moral imperative, however; despite the motivation provided by American incompetence Nixon's stance was relatively admirable. Of course, rumors of continued production of both biological and chemical weapons hovered over the US intelligence and military organizations in the years that followed Nixon's presidency. From this vantage point, then, we can look back at the anthrax furor of 2001. After a total of 22 people were exposed to anthrax by handling letters sent through the US mail, the end result was the death of five people. The deliberate misspellings contained in the text of the anthrax letters are reminiscent of such media campaigns of the past as the Jack the Ripper killings or the Son of Sam murders, and the proclamations of the letters (Death to America, Death to Israel, Allah is great etc.) seemed right away to be an obvious attempt at provocation. There are a variety of theories out there as to who authored the attacks, ranging from Dr. B.H. Rosenberg's very public tarring of Dr. Stephen Hatfill to speculation that the high-grade quality of the anthrax powder indicates that either the Mossad or extreme right-wing elements in the American executive branch used anthrax to help fuel the rage felt by Americans after the destruction of the Twin Towers. Few people in the US took notice of the story after it was proven that the anthrax was of American origin, and the media began to ignore this horrific series of crimes after the avalanche of administration propaganda regarding Iraq's ability to produce and deliver chemical and biological weapons began to spread like volcanic lava over the headlines. Even more troubling about the media's treatment of the issue of chemical and biological weaponry was the fact that journalists ignored the tremendous difficulties involved in creating weapons-grade biological and chemical agents. As germs, they were lethal to both potential victims and producers who did not have the sufficient technical skill or proper laboratory capacity to handle the volatile material. Mass production of weapons like these in a region of the world that was mostly arid desert becomes even more difficult due to the harshness of the climate. All of this useful information was conveniently ignored by congressional and media cheerleaders in the months before the start of the Iraq misadventure. Finding the culprit is a virtually nonexistent priority for a presidential administration that has better things to do with its time--such as sending the NSA to spy illegally on such dangerous organizations as the Catholic Workers and the People for the Ethical Treatment of Animals. Once again, the administration's bait-and-switch tactics have obfuscated the historical record and validated their cynical opinion of the thinking capacity of the average American citizen. Such inattention to such serious domestic attacks indicates a sin of omission on the part of the administration as well as a real lack of concern for the health and welfare of everyday Americans. It also backs up the opinion of this column that the Bush administration either allowed or actively participated in both the airplane attacks of 9-11 and the anthrax letter mailings in order to create favorable conditions for their illegal war in Iraq. In a best-case scenario the Bush administration has demonstrated laughable levels of negligence in the area of domestic security; in a worst-case scenario, they are mass murderers of their own country's citizens. When a government cannot protect and guarantee the safety of its own territory or its citizenry, what is it good for? So this brings us back to the death of Otillie Lundgren, age 94. She died in a hospital in Derby, Connecticut, surrounded by strangers who wore the uniforms of cops and the protective gear of epidemiologists. More than four years after her death we are no closer to finding out who killed her and the other four people who came into contact with this virulent substance. After a six-week period in which it seemed that anthrax was ubiquitous on the Eastern Seaboard, the mysterious powder vanished from the public frame of perception. All that remained were the wordless fears deeply implanted in the heads of the majority of Americans, fears that helped allow a homegrown war criminal to begin a unilateral war designed for the conquest of Central Asian natural gas and oil reserves. Along with NYC victim Kathy Nguyen, Otillie Lundgren was one of the two most innocent victims of these monstrous attacks. Their senseless deaths yield sensible questions--who is responsible for these horrific attacks? And who profited the most by their deaths? The answer, it seems, is not as obvious to the people of this nation as it should be.

Raiders @ Patriots Fanatical Sophistry

Posted on August 05, 2008 in Impotence young men

This post will be posted at Harkonnendog, as usual, but also at Fester's Place. Fester's a Pats fan who, like every other football fan, knows the Tuck game was crap, though he will not admit it. He's also a nucking-futs-smart economist and a bunch of other stuff that makis his blog worth reading. Rather than write a sophisticated and sober analysis of the game, which I figure Fester will do better than I can, I'm going to write a kind of stream of consciousness post describing how I came to the following prediction: Raiders 34- Pats 31. It wasn't easy getting there, but now that I'm there I'm sticking to it! Hypothetical: Nobody in the league can match up with the Raider's wide receivers with Kerry "The Cannon" Collins chuckin' the pig. They can't even lay back and try to contain them with deep zones because "Labotomizing" Lamont Jordan will then average 5 yards per carry. Raiders win! Test: That Indy team has a pretty good QB, receiving corps, and runner. Since they wrecked the Pats we will too- oh shit. But: With Ty Law and Romeo Crennel gone the Pats aren't the same. Plus Moss is better than Owens and Owens had a huge SB. Result: Raiders will score a lot but not dominate the Pats defense. Hypo: The Raider D will contain the Pats offense. Test: I won't even bother to try to justify this. But: We're better. We had a great preseason against the likes of, um... we ARE better, though! Plus we have this freaky-deaky 6-5 lineup nobody has ever seen. Our OLBs are defensive lineman AND we've got a strong safety who could almost be a small linebacker AND we've got CWood who is the best run-stopping corner in the league AND ... well that's enough, ain't it? The Pats won't run on us. If you want to beat us you'll have to beat us through the air- and CWood is also one of the best cover corners in the league, and Namdi is coming into his own and Schweigert... er... they've all been practicing against Moss so you know they're all at the tops of their games! Result: The Raider D containst but does not dominate Pat's O.

Raiders @ Patriots Preview from Fester's Place

Posted on August 05, 2008 in Impotence young men

I am ready for some meaningful football. (My fiancee is now rolling her eyes over the last two words in that sentence... but still). Tonight sees the Oakland Raiders visit the New England Patriots at Foxboro for the kick-off game of the Pats' quest for three straight. I see this as a reasonable objective for the Patriots, but it will, as always be a difficult journey. I see the Patriots as a well managed team that attempts to use its versatility as its core competence. As I have written before, the Patriots seek to take away the opposition's first choice option. The fundamental gamble here is that the Patriots are betting that their second, third or fourth preferred style of play is vastly superior to whatever the opponent's second preferred style of play. This was seen most notably in the 2002 Super Bowl against the Rams where the Patriots were able to disrupt Marshall Faulk for the entire game, and thus putting the offense solely on Kurt Warner's shoulders. The Greatest Show on Turf managed 17 points. This year, I think that the Patriots are significantly deeper and more versatile on offense. With the return of Ben Watson, I will be shocked if the Patriots do not line up with two tight ends for at least forty percent of their first downs. Dan Graham and Ben Watson present some intriguing passing game match-up problems for any team that does not have multiple rover linebackers/strong safeties (6-2+, 225lbs+, 4.5 or better speed) and if a team has a light fast defense in, the tight ends can stay in and assist the power running game. This is one of the many less than pleasent choices the Patriots force teams to make. This is a key illustration of what Bill Bellicheck has been attempting to build over the past five years --- situations where the best solution is still an unsatisfactory solution for the opposition. I am slightly worried about the wide receivers over the course of the season as Deion Branch, he of the amazing quicks, and fragile body, has yet to play a 16 game season. Troy Brown is old, as is Tim Dwight, while Andre Davis is new right now. Bethel Johnson has amazing speed, but is still inconsistent and injury prone. The only receiver I am not worried about is David Givens. However within this set of receivers, there is a wide diversity of skills and attributes that would allow the Patriots to quickly shift their offensive focus from short crossing routes in a West Coast Flavor to a vertical game. I would think tonight that the Patriots would want to take advantage of the comparative slowness of Oakland's big linebacker corps and run plenty of crossing routes and seam patterns with receivers and tight ends isolated against a linebacker who is bigger than some defensive ends. Now onto defense, this has been the forte of the Patriots' ability to mirror and deny an opponent's strength over the past four years. I am liking the defensive line quite a bit. I was surprised that the Patriots cut Rodney Bailey in favor of keeping undrafted rookie Mike Wright, but even still, the Pats in their base 3-4 have significant talent in their starters and impressive depth. Richardy Seymour and Vince Wilfork each demand two blockers on running plays, and Ty Warren will dominate against most right tackles when he is one on one. Oakland will be forced to keep their running backs or tight ends in to help block more often than they would prefer. The strength of the defensive line is the strength of the unit as a whole. If the three linemen and the typical pass rushing linebacker can force Oakland to keep seven blockers in, the task for the secondary just got a whole lot easier. I am not sure who the starting cornerbacks will be tonight, as the Patriots currently have four corners who have started for Super Bowl winners on the roster. I would imagine that the Patriots will attempt to play some aggressive man with zone support against Randy Moss and keep Chad Scott in as the physical bump and run corner for at least third down plays. I also anticipate the Patriots keeping Eugene Wilson in deep centerfield for most of the game, as I think that the Patriots would be willing to see Moss have a T-O type game if they can shut down everyone else on the field. They just need to avoid the multiple big plays that are Moss's bread and butter, even if that means giving up more 10-15 yard gains. I am not sure what the Patriot linebackers will be doing this year. The outside linebackers are experienced, fast, smart and solid. Roosevelt Colvin should finally be completely healed from his hip injury suffered two years ago while Willie McGuinest just gets craftier and better as his health is still with him. I doubt that Willie McGuinest will be healthy for the entire season, but Tully Banta Cain, in limited playing time, has shown some significant pass rush ability over the past year. Mike Vrabel is just a playmaker who is never out of position. The loss of the top three playing time inside linebackers from last year is a large loss which creates a significant question mark. I am glad that Vrabel is shifting to the inside, as he is smart, and it will keep the Pats' three best linebackers on the field more often. The question mark is who plays next to him --- Monty Biesel or Chad Brown. I think this will be a platoon position with Brown in on run downs and Biesel in on pass downs. I have a difficult time seeing how the Raider's defense can keep the Patriots from scoring, and so far, the Patriots have taken down better quarterbacks who play vertical passing games with comparative ease (Hi there Kurt, hi there Peyton.) I forsee a lot of pressure, and a lot of sacks against Collins unless the Raiders invent an effective screen game. Therefore, I am calling it 27-17 Patriots winning.
