Valdiserri, CDC's director of HIV/AIDS prevention program. "Use of the rapid tests is key to the success of these efforts." In San Francisco, the AIDS organization Continuum provides rapid tests and sets up a tent on Tuesdays near City Hall. Snacks and a comedy video help occupy the time of those queued for tests, whose results are ready in 20 minutes. Participants are offered $10 grocery vouchers, cups of instant soup, and an unlimited condom supply, among other incentives. Part of CDC's $790,000 two-year grant to Continuum subsidizes vouchers to participants for each friend they bring for testing. Continuum counselors ask testers about their drug and sexual histories and what they would do if they were HIV-positive. Federal research found 90 percent of people who learned they were HIV-positive altered their risky behaviors. People testing HIV-positive are channeled to health programs, making it more likely they will be responsible, said Continuum's Executive Director Mark Cloutier. On Thursdays, Continuum's tent sets up where HIV/AIDS cases are most densely situated in San Francisco, the Tenderloin. 2007 Failed States Index Released

The Fund for Peace published its most recent list of failed states. Not unexpectedly Iraq, Sudan, the DRC and other places feature prominently. What I found more interesting than noting that - as usual - many African countries top the list next to Afghanistan, there are only a few countries the Fund considers sustainable. Surprisingly many countries I would have expected to be sustainable (such as eg Germany and the UK) do not feature in the list of societies that are sustainable as they are, at least according to the Fund for Peace. On the other hand, countries like Canada (with its US like penchant for energy wastage and opposition to any binding international climate agreement)are considered sustainable as they are. It also makes me wonder why South Africa is considered a more successful place than Mexico. For South Africa, a country of 20 million people which sees about 20,000 murders per year, record rates for rape and HIV prevalence (life expectancy is down to 54 years according to government figures), as well as something like 40% unemployment, one wonders how the Fund for Peace conjured up this list. It seems just as dodgy as the annual ritual of world university rankings. Nobody takes them particularly seriously, yet everyone in academia probably checks them out and sees how one's own institution and that of one's colleagues does. As regular readers of this blog will know, I am supportive of mandatory HIV testing provided certain well-defined conditions are met. Stuart Rennie seems to disagree. Here I reproduce his take on the issue. It's well worth reading. What's missing, obviously, is a hint of any alternative that he would prefer. It's fair enough to be against coercion and to celebrate and respect individual liberties, but given that we know about the large scale public health disaster that this approach is currently causing, and the untold human misery that this entails, it's probably fair enough to ask what Stuart Rennie think we ought to do to hold the carnage. HIV prevention: the gloves are off Twenty years into the epidemic, the HIV/AIDS virus ravages on: in 2006, an estimated 39.5 million people in the world were living with HIV, 4.3 million were newly infected, and 2.9 million AIDS-related deaths. Of the deaths, 2.1 million occurred in sub-Saharan Africa. As for new HIV infections, South Africa alone is estimated to have 1500 ... per day. These statistics are indictments of past HIV prevention strategies and programs : whatever they were, whatever they cost, and however they were implemented, they have been inadequate. The question then becomes: what strategy changes should be adopted? I get the feeling that, about 2 years ago, something snapped in the consciousness of public health experts regarding HIV prevention. Enough was enough. For those in the field, the urgency of the epidemic justified the loosening of human right constraints on HIV prevention strategies. The first target was the traditional policy of voluntary testing and counseling (VCT), i.e. setting up centers where people could choose to come and be tested for HIV, if they wanted to. Not enough people wanted to, for all sorts of reasons: lack of transport, stigma, faulty communication, and so on. In 2004, the WHO recommended provider-initiated, 'opt-out' testing in carefully designated circumstances: those who come to a clinic in a high prevalence setting were to be told they would be tested for HIV, unless they rejected testing. The CDC soon followed suit with similar policies. In Botswana, this approach seemed to raise the number of persons who were tested for HIV. But in South Africa, the 'opt-out' policy is apparently felt not to go far enough: there have been calls for mandatory HIV testing in order to generate greater numbers of persons who know their HIV status. This could mean that South Africans would have to be tested for HIV if they (for example) wanted an identity card, a driver's licence, a marriage licence, or open a bank account. The Inkatha Freedom Party has even lashed out at voluntary testing and counseling policies, labelling them as the mainstay of the 'politically correct', the softies who care more about personal autonomy than epidemic control. VCT, in other words, is for pussies. Not everyone is buying it, of course. Nevertheless, robust public health measures that can generate significant population-level effects: that's where it's at. Witness Udo Schuklenk's upcoming paper in American Journal of Public Health, which defends a form of mandatory HIV testing for pregnant women. Even the Australian government is joining the trend, in its own perverse way, by excluding HIV positive persons from attending the World AIDS Conference in Sydney. Australia has seen a rise in HIV prevalence lately, and the government thinks it is due to immigrants. Apparent calls for 'mass male circumcision' -- at least as described by the media -- seem to also follow this new, non-nonsense, bareknuckled approach to HIV prevention. Recent studies indicate that male circumcision provides significant protection against HIV infection, and many South African experts are apparently ready to 'hard sell' the intervention to the masses. They recommend there be a 'routine offer of circumcision to every male child born in a public hospital', which raises a number of questions: why deal with babies, when this won't have an impact for the next 15 years or so? How will communities respond to such aggressive policies? Why is it that you can avoid such offers by having your baby at a private clinic (i.e. being wealthy)? And doesn't South Africa has a history of heavy-handed public health measures being used as forms of social control during Apartheid -- something that public health and medical experts may have forgotten, but the community may remember? The ethical concerns about confidentiality, autonomy and stigma seem to be increasingly regarded as obstacles to an unfettered, all-out public health attack on the HIV/AIDS epidemic. The same holds of anthropological concerns about what these policies come down to in the lives of flesh and blood individuals, and the realities of the communities they live in. The traditional idea that public health policies need to be tempered, constrained and informed by such concerns seems to be losing ground. I don't know why people dig him so much.) Most modern Christians read the Bible, in fact they STUDY the Bible, and I don't think that was always so. Certainly that was not true back in the day when nobody could read the Bible because it was in Latin. Reading the Bible doesn't really lend itself to anti-Semitism. Come to think of it, there may be a pretty direct relationship between the percentage of Christians who read the Bible, and the lessening of anti-Semitic violence. And as long as I'm thinking with my fingers, (by typing I mean) I have to say that 400 years ago Christianity was a lot more like Islam is in the Middle East today. A bunch of assholes were in charge of the religion AND the government, and when people got pissed off at the religion and/or the government they blamed the Jews. Hmm... But Muslims today read the Koran, so the theory breaks down. Or do they? How many Muslim in Saudi Arabia actually read the Koran? Let's assume all those who CAN read do read it. But her cookbooks brought her national recognition. Along with "The Taste of Country Cooking" in 1976, she wrote "The Edna Lewis Cookbook" in 1972 and "In Pursuit of Flavor" in 1988. She and Peacock wrote "The Gift of Southern Cooking" in 2003. "Edna was a very important voice for her knowledge of Virginia-style Southern food and cooking," Judith Jones, Lewis' editor at Alfred A. Knopf publishers, told The Times in 2003. "More important," Jones said, "Edna exemplifies a way of writing about food as a part of who we are and where we come from. It is food writing as memoir." Some food experts referred to Lewis as the leading African American female chef. Others placed her as the dean of all Southern cooking. Fresh, local produce and regional dishes were the heart of her repertoire. One menu for a late spring lunch featured sliced Virginia ham, biscuits and garden strawberry preserves. "Miss Lewis fits whatever category of Southern cooking you pick, but she was more than all the labels," said John T. Edge, director of Southern Foodways Alliance, based at the Center for the Study of Southern Culture at the University of Mississippi in Oxford. In several of her books, she wrote about her early years in Freetown. Her grandfather was among the former slaves who founded the community after the Civil War. Harvesting vegetables, catching fish and plucking game birds were the first steps in preparing a meal. "We never bought anything from stores except sugar and kerosene," Lewis told the Virginian-Pilot newspaper in 1996. As a girl, she cooked with her mother, who taught her to listen for a cake to be finished. "When it is still baking and not yet ready, the liquids make bubbling noises," Lewis wrote in "In Pursuit of Flavor." Lewis' father died when she was 9. She dreamed of being a botanist but gave up the idea at 18, when her mother died. She moved to New York City looking for work in the early 1940s. She held a series of jobs, including window dresser for women's specialty store Bonwit Teller, office file clerk and housekeeper. She often cooked for her friends. One of them, John Nicholson, owned an antique shop. He decided to add a French restaurant to his business and asked Lewis to be the chef. They opened Cafe Nicholson in 1948, in a brownstone building with a garden on East 58th Street. Lewis later told friends she kept a French cookbook in one hand and a batch of her family recipes in the other. "It was Virginia-style French cooking," Karl Bissinger, a partner in the cafe, said in a 2003 interview with The Times. "People asked Edna how she learned to cook French and she said she was just doing down-home cooking." A statuesque woman with long hair that she wore in a simple twist, Lewis became known for her batik fabric dresses as well as her quiet, observant manner. She rarely spoke of her personal life. She was proud of her heritage but showed it in subtle ways, Jones said. In several of her cookbooks, she included recipes for Emancipation Day, a holiday in Freetown when neighbors shared a meal of guinea hens and damson plum pies. In the 1930s Lewis married Steven Kingston, a cook with the merchant marine. They were political activists who joined the Communist Party. "I was a radical," Lewis told Bon Appetit magazine in November 2001. She worked in the office of the Daily Worker, the Communist newspaper. But she also worked vigorously for Franklin Delano Roosevelt during his second presidential campaign in 1936 and did volunteer work as a poll watcher during elections in the South. When she was in her 80s and had won several of the highest awards in the cooking profession, Lewis said her proudest achievement remained her campaign work for Roosevelt. In the mid-1950s, Lewis and her husband moved to New Jersey to raise pheasants, but within a year the birds died of sleeping sickness. Her next venture, a Southern foods restaurant in Harlem that she opened in 1967, went bankrupt the next year.
